AFFIDAVIT

I, Erica Paraso, 35 years old, single, Filipino, with residence

at Sitio Balon, Brgy. Lawin, Tarlac City, after having been duly
sworn to in accordance with law do hereby depose and say that:

1.

I am a medical doctor since 2000 at Central Luzon

Doctors Hospital, Brgy San Sebastian, Tarlac City;

2.

That also sometime October 18, 2015, there was a patient

who was rushed into the emergency room wherein one
woman in the name of Ms. Jaimie Tan was shown to have
blood on her body and bruises and abrasions on her face;

3.

I then examined her and based on my findings, she

suffered trauma, contusion on her left arm and leg,
abrasion in her left leg and a cut on her right eyebrow
and upper right lip. She also suffered severe bleeding
which caused the unborn child. This is specifically
evidence in the Medical Certificate that I issued to her
and is hereto attached as Annex 1 and made an
integral part hereof;

I asked Ms. Jaimie Tan that time what had happened but
she refused to answer as if she did not want to answer
anything at all;

5.

I even asked the companion of Ms. Jaimie Tan that time

who is Mang Rolly and said that he only saw the patient
inside her house already unconscious which led him to
bring her immediately to the hospital;

6.

Last November 1, 2016, I was also the attending

physician when again, Ms. Jaimie Tan was rushed to the
emergency room wherein I declared her dead on arrival
since she suffered an over dose of medicine. The Death
Certificate with diagnosis is hereto attached as Annex
2 and made an integral part hereof;

7.

That I am executing this affidavit in support to the

Opposition to the Petition filed by MS. MONSHER TAN.
