Eye Sight Trust is a registered non-profit, non-political, and non-governmental

organization. Its prime objective is to provide free or affordable treatment to the
poor patients coming to Khalid Eye Clinic and also to facilitate treatment in
Karachi and rest of the Pakistan. Eye Sight Trust is now in need of funds to help
support its noble cause it has been pursuing for over a decade.

Meezan Bank Response

o They welcomed us, and mentioned the following four documents that need
to be submitted before charity application can be processed:

A cover letter or a proposal

Registration certificate of Eye Sight Trust
Audited accounts and reports of Eye Sight Trust
Tax exemption certificate duly issued by FBR
