Authority to release information under the Data Protection Act 1998

I authorise you, Verifile, and your representatives to:
carry out checks on my employment history, education, professional qualifications and membership C H E C K I N G F A C T S
of professional organisations;
BUILDING TRUST
take up character/professional references;
confirm the information I have provided to my employer, to Verifile and information I have included on my CV
or other application for employment; and
carry out any other background checks that are necessary, including a credit check and criminal record check.
I give all previous employers, educational institutions, professional institutions, credit agencies, government organisations and others
permission to give you and my current or new employer (or people acting for them), information about my background (including
information about my driving records, credit records, criminal records and other public records) to you and to my current or
prospective employer or people acting for them, in the European Union and, if appropriate, other countries regulated by data
protection directives.
I also authorise you and your representatives to give personal information about me to other organisations if this is necessary for you
to carry out the checks. I agree to my personal information being transferred outside of the European Economic Area if this is
necessary to complete checks. I also authorise organisations in any country to give my personal information to the organisations
referred to above, and my employer, as long as this is in line with relevant laws.
If I have previously lived in the UK, I give you, Verifile, and your representatives the following permission with respect to my criminal
record check in the UK:
obtain information on criminal convictions I may have which are unspent according to the Rehabilitation of Offenders
Act 1974;
use my personal information, including anything that is sensitive;
contact me direct to get any information you need to carry out basic-disclosure checks on me and to check my
information before passing it to my current or new employer (or people acting for them);
give my information to other organisations, such as local and central government departments, the police, courts and
tribunals and legal advisors; and
receive a basic-disclosure certificate on my behalf and pass it to my current or new employer (or people acting for them).
I also give my current or new employer (or people acting for them) permission to:
keep the basic-disclosure certificate for up to three months; and
safely get rid of the basic-disclosure certificate after this period, without making any copies.

By completing this consent form Verifile will check your details with / against fraud prevention databases. Should Verifile or
your current / new employers investigations identify fraud or the commission of any other criminal offence by you [on your
part] when applying for, or during your employment with your current or new employer, your current / new employer will
record details of this on relevant fraud prevention databases. This information may be accessed from the UK and other
countries and used by law enforcement agencies, by current or new employers and other organisations to prevent fraud.
Please contact the Human Resources department at your current or new employer if you want to receive details of the
relevant fraud prevention databases through which the employer shares information.
By signing below I confirm that I have read and understand your privacy policy (shown on the website at www.verifile.co.uk/privacy)
about handling personal information. I understand that in some circumstances you may continue to carry out searches during the
course of my employment and I agree to this.
