DECLARATION / UNDERTAKING

In consideration of the training and other facilities to be received by me from Central

Scientific Instruments Organisation, Chandigarh. I .. Son/Daughter

have read all the rules/regulations of the training and hereby undertake the following:1. I

shall:
Strictly stick to the work/Project assigned
Get the Mid Term Approval of the work done and submit /formal report
to the DU Incharge to the Head BDG
Submit the detailed technical report at the end of training
Be properly dressed during the training period
Not Wander at the work place without permission
Be regular & punctual
Adhere to time, and shall not leave the workplace without permission
Not use any drugs, alcohol or cigarettes etc.
Not cause any willful damage to any lab. assets
Not cause any damage to any equipment or machinery belonging to
the organizations arising out of the negligence of duty on my part, otherwise
the same shall be recoverable from me

2. Scientist Apartment:

I shall be personally responsible for any loss of personal belongings. CSIO

will have no implication legally or otherwise.

In case of any damage to property in Scientist apartment, I authorize CSIO to

recover such losses from me

I shall use only the Locks provided by CSIO. In case I use my personal
locks, CSIO is authorized to break such in any warranting situation or
emergency and will not be held responsible for any kind of loss

In case of any problem in my room, I hereby authorize CSIO to open the room
for redressing the problem

I shall maintain absolute secrecy regarding the progress of work, operational
reports and statistical information etc. concerning the organization and not
communicate the same to anybody without written permission from the
Organization

CSIO has the right to terminate my training summarily, in case I am found
guilty of :
a)
not observing the rules and regulations
b)
non-satisfactory progress at any stage, or
c)
misconduct
5.

I also undertake that during the period of my training, I shall observe the rules
and regulations of the Organization and absolve CSIO from any kind of
responsibility for any accident etc. or injury sustained by me during my period
of stay at CSIO.
