The Fine Print

All plans are valid for one year from date of sign-up.
Patient agrees that plan fees stated must be paid at time
services are rendered.
Any service not paid at the time of service will be billed at
usual and customary fees (unless prior arrangements are
made.)
Plan fees are valid only when paid at the time of enrollment.
All family members must reside in the same household (or a
full-time student under 23).
A 10% Service fee will be added if patient wants to pay
monthly through Care Credit for any discounted services.
Care Credit may not be used to purchase the Dental Savings
Plan (dictated by agreement terms established by Care

Credit).
