The undersigned would like to request that he be transferred

from his current station, SAN MIGUEL NATIONAL HIGH
SCHOOL ( SMNHS) to MONDRAGON AGRO - INDUSTRIAL
SCHOOL (MAIS) for compelling reasons to wit:

1. Imminent threat against his person.

2. Armed conflict in the area.
3. Health deterioration on account of the long trek /walk to
the station.

By transferring to the herein mentioned school, the

undersigned believes that he will be able to better function
as teacher-educator in service to the students. He looks
forward to your positive response as regards this request.
