AFFIDAVIT OF LOSS OF TRANSFER CERTIFICATE OF


TITLE NO. 224948
I, AIDA P. CARIO, of legal age, Filipino, married, and resident of #13 Ascalon St.,
BF Resort Village, Las Pinas City, after being duly sworn in accordance with law, hereby
depose and say that:

I am the atty.-in-fact of ASUNCION P. LAYOG, as per Special Power of Attorney


executed on March 31, 2010 before Daet, Camarines Norte Atty. Reynaldo Paguirigan
Jr., under Doc. No. 63, page No. I, Book No. XI, series of 2010.

2. My sister ASUNCION P. LAYOG, is the registered owner of a one parcel of land


situated at Town and Country Hills, Antipolo City, and covered by a Transfer Certificate
of Title No. 224948, the parcel of land is more specifically described as follows:

A parcel of land (Lot 32, Blk. 18 of the cons. subd. plan, Pcs-04-002153, being
a portion of the cons. of Lots 2-A to 2-H & Road Lot 2 (LRC) Psd-292499, LRC Rec.
No. ), situated in the Mun. of Antipolo, Prov. of Rizal, Bounded on the NW., along line 1-
2 by Lot 33, on the NE., along line 2-3 by Lot 30; both of Blk. 18. Xxx, containing an
area of FIVE HUNDRED FIFTY SEVEN (557) SQUARE METERS. xxx

3. I am in possession and custodian of said original owners copy title which my


sister entrusted to me, for me to be able to sell the said parcel of land.

4. On or about February 5, 2014, I went to Antipolo City to make payments for


the real tax, I brought the original owners copy of said title to have it photocopied, the
said owners duplicate was inside a brown envelope.

5. Upon reaching home in Las Pinas City, I discovered that the brown envelope
containing the aforesaid original owners copy of said Transfer Certificate of Title no.
224948 was lost/missing.

6. In spite of all diligent efforts to look for said original owners copy of said
Transfer Certificate of Title no. 224948 , all my efforts proved futile and in vain, thus, I
am now declaring the loss of the said original certificate of title.

7. The said owners copy of Transfer Certificate of Title No. 224948 was
inadvertently lost by me.

8. Said Transfer Certificate of Title no. 224948 was never given, sell, pledged,
mortgaged to anybody, nor allowed anyone to use for any transaction or disposition,
there being no such transaction.

9. I am executing this affidavit to attest to the truth of all the foregoing facts,
documents and the contents thereof for all its legal effects, intents, consequences and
purposes, including to request the Office of the Registry of Deeds to annotate this
affidavit of loss and for proper action, pending the petition with the court for the
issuance of another original owners of said title certificate in lieu of the one that was
lost.
