I. Premium
A. The premium is based on an estimated annual payroll and is subject to audit at policy expiration.
B. The deductible premium is calculated by multiplying the DISCOUNTED Classification Rate x Final Audited Payroll.
C. The deductible premium is payable with a down payme 25% and # of installmen 8
D. The policy is subject to a Minimum & Deposit Premium.

II. Auditable Policy


A. Increases in payroll will increase the Deductible Premium.
B. Increases in payroll will increase the "Basket Maximum" (Aggregate Loss Fund Amount).

III. Paid Loss Program


A. The policy does not convert from paid losses to total incurred losses at anytime in the future, unless so requested by the Insu
and agreed upon by the insurance company. All claims remain open until fully closed. Claim payments only apply to current
medical bills and final disability/settlement payments (not reserved claims).
IV. Loss Payments
A. Policyholder is responsible for all loss payments up to the Deductible Amount and up to the "Basket Maximum" Final Audited

V. Letter of Credit Requirement


A. A pre-determined amount of collateral is calculated based on projected losses ( Loss Pick) for the current year. If the program
been in effect for more than one year the LOC will be adjusted for older years based on Paid Losses + Reserved Losses and a
Incurred But Not Reported (IBNR) Factor will be applied against outstanding losses. The LOC must be drawn on an approved
bank and must comply with NY Regulation 133 for letters of credit. See Security Section of This Proposal.

VI. Escrow Fund


A. A pre-determined amount of money will be held by the insurance company to pay on-going claims. The amount is calculated
on a three month average.

A. Bodily Injury By Accident

Loss arising out of one loss event (accident) that exceeds the Deductible Amount will be paid by Specific Excess Insurance
If there is more than one person involved in One Accident only one deductible applies (except loss by disease).

B. Bodily Injury By Disease

All loss arising from disease to each employee that exceeds the Deductible Amount will be paid by Specific Excess
Insurance. Disease claims are subject to a separate Deductible applying to each person that is injured by disease.

C. Employer's Liability

All claims that fall under Employer's Liability are subject to a Per Claim Deductible.

D. Deductible Amount

The Deductible Amount(s) is the most you must reimburse the insurance company for indemnity and medical benefits,
damages and Allocated Loss Adjustment Expenses combined for bodily injury to one or more employees as the result of
any one accident.

The Deductible Amount(s) shown for Bodily Injury by Disease is the most you must reimburse the insurance company
for indemnity and medical benefits, damages and Allocated Loss Adjustment Expenses combined for disablement of one
employee due to Bodily Injury by Disease.
