1.0 Confirmation of Previous Meeting Minutes Action

The minutes of previous meeting No. 2 was confirmed without amendment. Info

2.0 Preliminary Matters

2.1 All Consultants issued 9 sets of Contract drawings to JIA. SH to revert to JIA on the collection by 10 SH
July 2015.

2.2 HK to submit breakdown of micro construction program for Phase 1 and 2 to all for comments by HK
15 July 2015.

2.3 HK installed the DANGER and NO ENTRY sign in 4 languages around hoarded areas and project Info
signboard at front entrance.

2.4 HK prepared fire evacuation layout plan and post at all staircases and along escape route. Info

2.5 HK to install noisemeter and settlement markers at adjacent buildings by mid July 2015. RTO to HK/RTO
monitor report. HK to submit noise monitoring and ground settlement report by next meeting.

2.6 HK to issue letter to SANH and Silra Home and cc Sunlove Home and consultants of the working HK
hours by 7 July 2015

2.7 HK to display on site office the insurance, factory permit, construction programme, pre-con survey HK
of surroundings, safety management plan and Risk Assessment Report

2.8 HK informed hoarding around Admin block to be completed by this week, 10 July 2015. HK

2.9 HK to submit name list of workers to SH for record by this week, 10 July 2015. HK

2.10 HK installed sub meter for water. HK to tap electrical facilities from spare capacity from main switch HK
room to verify with LEW of Sunlove Home.

2.0 Preliminary Matters Action

2.11 HK submitted to PUB earth control measurement; pending approval. HK

2.12 HK to submit schedule of specified materials finishes and samples by 15 July 2015. HK

2.13 HK to submit schedule of scale shop drawings, PEs submission and certifications and method HK
statement by 15 July 2015.

2.14 HK to highlight major findings discovered from pre-con survey to Silra home for their info. HK

2.15 HK to survey existing building and services before commencement of construction works and to HK
update all by next site meeting.

2.16 RTO started working since 1 July 2015. HK to provide RTO with site facilities and submission forms HK/RTO
e.g. internet connectivity, site memo, inspection form and weekly record. All the correspondences
should keep SH and consultants in the loop.

2.17 HK completed site facilities, ie Generator, ECM, Site Office, Meting Room, RTO Office, Guard Info
House and Toilets at rear entrance Gate 3 and provided noise barrier hoarding around generator.

2.18 HK completed temporary guard house and visitor's waiting area. SH has moved in. Info

2.19 SH approved HK proposed relocation of existing M & E system in temporary Guardhouse, to be HK


completed by this week

2.20 HK presented the 3 additional gates, Gate 1, 2 and 3 to IMH with the presence of MOH and SH on HK
2 July 2015. IMH, Joseph Tsu is OK for heavy vehicle coming in/out of Gate 1, however ejected the
temporary and required HK to appoint professional traffic consultant to assess the impact of the
construction activities and traffic flow and circulation within the IMH vicinity.
SAC to arrange with 3 Traffic Consultants to meet for discussion with HK to call for quotation by
next week. Appointed Traffic Consultant will be directly under HK.

2.21 HK sent letter to adjacent neighboring facilities i.e. IMH, SILRA Home and SANH to broadly inform Info
them of the following before commencement of works:

1) The estimated construction period of Sunlove Home (start and end MM/YY)
2) To seek their understanding on any noise created and increased traffic flow of
construction vehicles during this period
3) A point of contact which they can be in touch with for any feedback relating to Sunlove
Homes expansion

2.22 HK to tabulate dimension and area of Silra Home to forward to MOH for SLA approval; if required. MOH
HK to also submit cost estimate of reinstatement works at Silra Home and email to all. SH and HK
to provide undertaking letter for reinstatement works. MOH to facilitate next meeting for further
discussion with Dr Seow and Silra Home.
MOH informed SLA required 2 weeks; to revert on decision by 13-16 July week.

2.23 HK submitted layout plan to mark out no. of temporary carpark lots required along Buangkok View MOH
driveway before pedestrian footpath towards IMH for MOH's approval. Pending SLA to evaluate
cost for parking lots and TOL rates of the additional open green areas requested by HK by 13-16
July week.

3.0 Progress of Works

3.1 HK highlighted critical items so that mobilization of heavy equipments can commence Info
HK informed the 3 listed items as determining factor for delay; whichever items takes longer for
approval.
1. Re-design piling works (estimated 6 weeks)
2. Access Gate 1 - to obtain Silra Home approval
3. Access Gate 2 and 3 - to obtain IMH approval

3.2 HK informed almost 70% of piles are affected. SAC informed HK to proceed with construction HK
works of those piles that are not affected.

3.0 Progress of Works (Cont'd) Action

3.3 SAC informed HK to state clearly the reasons for the delay to submit re-design piling works. HK

3.4 HK to propose 30% more RC micro piles. HK to absorb re-design of piling works. Info

3.5 HK to resubmit emergency assembly point plan and post at all staircases and along escape route; HK
to differentiate color arrows for residents, staffs and workers. HK to ensure fire hydrant shall be
accessible. HK to put up signage at hoarded area to inform fire hydrant placement.

3.6 HK to forward emergency response team to Sunlove Home guard and put up at site office. HK

3.7 HK submitted for temporary toilet clearance; pending approval from PUB sewerage HK

4.0 C & S Matters Action

4.1 HK to submit technical controller, piling specialist to CV by this week, 10 July 2015. HK
Demolition Method statement and ERSS Design this week; to obtain approval in 2 weeks time.

4.2 HK to submit 1 set of re-engineering/design piling works to CV and the other set to AC HK
simultaneously by 15 July 2015. CV to submit to BCA which may take up to 2 weeks for approval.

4.3 CV approved concrete mix design. Trial mix inspection to be conducted on 9 July 2015 RTO

4.4 Permit of demolition works received on 3 July 2015. HK to display Permit in site office. HK informed Info
they required 3 weeks for demolition works.

4.5 CV submitted Pre CCTV survey of last IC to minor sewer; to forward acknowledgement letter from CV
PUB.

4.5 HK to highlight discrepancies among consultants' drawings early. HK

5.0 M&E Matters

5.1 HK completed 80% of trial holes; remaining can only be conducted after demolition of buildings. HK HK
to highlight findings.

5.2 HK to inspect unknown signal after demolishing existing guardhouse; to conduct additional trail hole HK
to determine unidentified signal.

5.3 HK conducted hydrostatic test and submitted report. HK


HK informed cracks findings at existing IC Line. HK to provide quotation for rectification works for
Sunlove Home consideration. HK to conduct CCTV to find out root of problem if appointed.

5.4 HK completed fire protection pre-con testing. Info

5.5 HK to prepare and submit equipment and material submission forms, catalogues, technical HK
specifications for review, comments and/or approval by 15 July 2015.

5.6 HK to superimpose underground drainline/IC on existing plans to highlight clashes. HK

5.7 TY to submit sanitary plan to CBPU early. TY

6.0 QS Matters

6.1 All progressive claims are to be copied to all consultants for verification before recommendation to HK
SH. HK to submit the format of progress claim for QS to comment.

7.0 Next Meeting

7.1 All parties agreed to held a site bi-meeting weekly to monitor closely on the progress of works. All
Next site meeting shall be on Monday, 20 July 2015 at 2:30pm at Sunlove Home, Site Office.
