SIPOC Form Guide

Note: This Toolkit is meant as a supplement to BPI's Lean Six Sigma training
courses. Usage of these tools without the training and appropriate context
may lead to errors.

Purpose
Document process boundaries and sets scope of project to ensure you are not boiling the ocean
Identify true customers to the process & the prioritized, quantifiable requirements (metrics) the customer places on the process
Helps identify and balance competing customer requirements
Aids in the identification of data collection needs in terms of requirements not currently measured
Identify who supplies inputs to the process
Develop questions for Critical Thought Map

Phase 1 :
Good: Establishes the start & finish of the process clearly; accurately defines the process in scope;
pertains to a process that provides value
Bad: Serves as a multi-step process documentation; scope is too broad
Phase 2 :
Good: Requirements are metric-driven and prioritized
Bad: Generic requirements are used - i.e. saying "timely" vs. received within 4 hours
Phase 3 :
Good: Identifies all relevent suppliers to the process that are pertinent and potential sources of variation
into the process
Bad: Suppliers are too broadly described and requirements from the process are too generic
into the process
Bad: Suppliers are too broadly described and requirements from the process are too generic
