1. Present this statement when paying at any BDO Unibank, Inc. branch counters. The machine validation will serve as your proof of
payment.
2. Please pay the Amount Due/Minimum Amount Due as indicated in this statement on or before your Payment Due Date to avoid
late payment charges.
3. For check payments*, please make checks payable to BDO Unibank, Inc.. Indicate the following at the back of the check:
