Graduation Memorandum
Congratulations! You may be a candidate for the September 30, 2016, December 31, 2016, March 31, 2017, or
June 2, 2017 graduation conferral date and may be eligible to participate in June 2, 2017 UMHS Commencement
ceremony in New York City. In order to be a candidate for graduation AND to receive your diploma and/or
transcripts please review the following guidelines.

Additional memos with a ceremony day itinerary and information on how to purchase additional tickets will be
forthcoming in late March 2017. Graduates will be provided with two, or possibly four complimentary tickets,
based on the number of people attending the ceremony. If available, a limited number of additional tickets will
be available at the cost of $50.00 per seat.

Graduation Application
All eligible graduates must complete the Graduation Application through the UMHS Student Portal
(http://www.mednurse-sms.com/estudent). Please log on to the student portal and go to the Graduation
Application link. Fill out the form, submit, and pay the graduation fee through the payment weblink. If you have
any questions, please contact the Registrar at Registrar@umhs-sk.net. You will not be considered for graduation
nor will a degree audit be completed without receipt of the Graduation Application and Graduation Fee. Please
note that the one-time graduation fee is a requirement for all anticipated graduates, whether or not you attend the
ceremony. You will be required to submit an updated Graduation Application if any of the provided information
changes, including address, graduation date, name, etc.

PLEASE NOTE: Diplomas will only be ordered for those students who have submitted the Graduation
Application and Fee in full by the due date listed below. If you are not able to submit an application and/or
fee by the listed deadlines or if you will NOT be graduating by the one of the listed graduation dates, please
contact the Office of the Registrar via email to Registrar@umhs-sk.net stating the reason.

For anticipated 9/30/2016 graduates, please submit the Graduation Application and fee by September 15, 2016.

For anticipated 12/31/2016 graduates, please submit the Graduation Application and fee by November 30, 2016.

For anticipated 3/31/2017 and 6/2/2017 graduates, please submit the Graduation Application and fee by
March 31, 2017.

Name on Diploma
Please list your name on the Graduation Application exactly as you are registered with ECFMG. If this name is
different from how you are listed on the UMHS database, you will need to submit certified copies of legal
documentation of your official name to UMHS. Name discrepancies will result in licensing and certification
problems.

Graduation Requirements
To be a candidate for graduation and receive your diploma and/or transcripts, you must meet the following
requirements:

Successfully completed all required Basic Science courses.


Successfully completed all required Clinical Clerkships. All final and original clinical grade evaluations must
be received. A fax copy of your clinical grade evaluation will NOT clear you for a diploma release.
Contact the Assistant Registrar for Clinical Grading for follow-up regarding grade evaluations.
Submitted all final and original administrative documents to the Office of the Registrar.
Submitted a passing score on the USMLE Step 1, Step 2 CK, and Step 2 CS to the Office of the Registrar
within 30 days of receipt. You may email, fax or mail the score report and performance profile if you have
not done so already. ECFMG does not automatically send scores to UMHS.
Apply for graduation through the student portal by the deadlines listed in this memo.
Pay the one-time $500.00 Graduation Fee when submitting the graduation application, payable via mail to the
Office of the Registrar or through the UMHS payment link on the student portal or UMHS website. This fee
is required of all students and is not contingent on attending the Commencement Ceremony.
Satisfy all outstanding balances owed to the University.

Graduation Date
Students completing the above requirements between:
June 2, 2016 and September 30, 2016 will receive a diploma dated September 30, 2016
October 1, 2016 and December 31, 2016 will receive a diploma dated December 31, 2016
January 1, 2017 and March 31, 2017 will receive a diploma dated March 31, 2017
April 1, 2017 and June 2, 2017 will receive a diploma dated June 2, 2017
If anticipated graduates are scheduled to sit for the Step 2 CK and/or CS after March 31, 2017, you
must show proof of the sitting date in order to attend the June 2, 2017 ceremony. Should you fail
the exam and subsequently have your anticipated graduation date moved to after June 2, 2017, you
may still attend the ceremony on June 2, 2017 but will not have your name listed in the ceremony
program until the 2018 ceremony program.

Graduation date is determined to be the first available graduation date after the successful completion of all
required clinical rotations AND passing the required boards. Note that if the boards are taken after the completion
of all rotations, the sitting date of the USMLE is the date used upon receipt of passing results. For instance, if you
sit for the boards on May 31st but your passing results are not received until June 30th, your graduation date would
be June 2nd if all other requirements have been met.

Graduation Clearance & Diplomas


Please note that your diploma cannot be released to you or ECFMG (even if you have attended the ceremony)
until the following is completed:

We have received all final, official clinical grade evaluations. The Assistant Registrar for Clinical Grading
makes every effort to track missing evaluations, but it is in your best interest to follow-up on all grade
evaluations. Please go to Student Portal (http://www.mednurse-sms.com/estudent) to view your clinical
rotation schedule and track receipt of your grade evaluations. A transcript with degree awarded will not be
released to ECFMG or a third party without receipt of original grade evaluations. A fax will not suffice for
grading or clearance.
Satisfy all outstanding balances owed to the University.
We have receipt of all required administrative documentation.

You have clearance for graduation by an internal final degree audit. An internal final degree audit
includes receipt of all the above-mentioned criteria and a file review of all evaluations. A final degree
audit will be conducted automatically when all requirements are met. Please do not call to request one.
Under no circumstances will a graduates diploma be released before a graduation date.

Special Graduation Dates


You may be eligible for a special graduation date if you meet one of the below criteria:

You are an International citizen and the USMLE Step 3 is a requirement in order to obtain a visa to begin a
U.S. residency. Submit your graduation application through the Student Portal, and email a copy of your
passport to Registrar@umhs-sk.net to verify your International citizenship.

You have obtained an off-track residency position, which is scheduled to begin within one month of your
scheduled graduation date. You must submit a copy of a signed contract with your Special Graduation Date
request.

A special graduation date will not be given for any day other than the last day of the month in which you
complete the curriculum and pass the required boards. To apply for a special graduation date, submit the
graduation application, along with supporting documentation, to the Office of the Registrar at least 2 months prior
to the date you are requesting. Your request must include the anticipated date you will complete all graduation
requirements. You will be notified via email if you are approved.

Diploma Distribution
Graduates cannot receive a copy of their diploma or final transcripts prior to the diploma date and final
degree audit clearance.

Original diplomas for all graduates will be mailed to the address provided on the Graduation Application after
your graduation date has passed and the degree audit has been completed. When submitting your Graduation
Application be sure to provide a permanent address where your diploma should be mailed. Your diploma will
automatically be mailed to the address provided after you have been cleared for graduation. If your address
changes prior to your receipt of your diploma, please submit a new graduation application through the
student portal indicating the new address.

Honors
Honors designees will be published in the commencement program and students will have their honors status
printed on diplomas. Eligible honors candidates will be acknowledged during the ceremony. The student with the
highest cumulative GPA, who is not a transfer student, will be invited to lead the graduates in the recitation of the
Hippocratic Oath. Due to the timing of receipt of evaluation grading and degree audit clearance, you may not be
recognized at the ceremony and the commencement program.

To be eligible for graduation Honors status, you must be a student in good standing, have a file review for
academic performance, have not have received an F in any course during medical school, have not sat for
any USMLE step in more than one attempt, and meet one of the following combined basic science and
clinical curriculum cumulative GPA requirements. The Honors designation is as follows:

ECFMG Certification
After a final degree audit is completed on or after the graduation date, a copy of your diploma will be submitted
to ECFMG for certification. Your ECFMG Certificate should arrive to your address on record with ECFMG
approximately 4-6 weeks after ECFMGs receipt of the diploma copy.

Additional information regarding graduation ceremony, tams, gowns, invitation, and hotel suggestions will
follow. All graduation notification and correspondence will be done through UMHS email. No other email will
be used.
