EMERGENCY ROOM POLICIES

1. All patients regardless of creed, financial and economic conditions must


be given all emergency procedures, drugs and stat laboratory diagnostic
examinations.
2. All patients brought to the emergency room must be attended
immediately.
3. Admission of patients should be done at the emergency room where the
patient is being examined, pertinent data and initial vital signs are taken;
insertion of IV fluids is being done and emergency treatment are given
before the patient is transported to the ward. This should be done by the
physician on duty.
4. Physicians on 24 hours duty (ROD) and emergency Room Nurse should be
available immediately for emergency.
5. Nurses assigned in the emergency Room have the full responsibility over
all drugs, instruments, equipment and supplies in the emergency room,
including patients still in the Emergency Room.
6. Nurse must endorse all supplies, equipment, instrument and drugs in the
Emergency Room
7. Chief Nurse will relieve the Emergency Room Nurse in case the Emergency
Room nurse is off Duty.
8. No one is allowed to open the Emergency Room cart (E-Cart) without
permission from the emergency Room duty nurse. All medicines to be
taken must have prescription from the Physician ordering it.
9. Recording in the Emergency Room admission book referral logbook and
pertinent data on the OUT PATIENT DEPARTMENT CARD when not busy is
the responsibility of the Emergency Room Nurse on duty.

EMERGENCY ROOM PROCEDURES

OUT-PATIENT CONSULTATION/ ROD (non-emergency/ non-admission cases)


1. After Patients pertinent data, vital signs and filling-up of patient chart
has been done from the OPD, patient record is then brought to
emergency Room for consultation / Evaluation done by the Resident
Physician on duty.
2. Resident Physician examines, determines and evaluates patient need.
3. Once patient is not for admission patient chart is then sent back to the
OPD for prescription and instruction of Home Medication
For ADMISSION:

Admission Procedure/ Emergency Cases

Step 1

Patients for admission must proceed to the Triage Area for evaluation, assessment
and registration, or to the treatment area for emergency management. For patients
with Direct-To-Room Order (Admitting Orders)

Step 2

The patient's significant other should claim the Patient Data Registration Form from
the Triage Nurse before proceeding to the Admitting Section for the processing of
the admission (choosing of room, signing of Patient's Data Sheet, filling up of HMO
Agreement Form or making a partial payment, getting the Admission kit, etc.).

Step 3

Go back to the Emergency Room and give the patient's chart to the Nurse-in-Charge.

1. Resident Physician examines, do Physical examination and history taking and


orders for admission.
2. Emergency Room nurse have Patient sign consent and carry out Doctors
Orders
3. Administers venoclysis, gives stat order medications and request prescribed
laboratory request.
4. Notifies ward of the new admission and consequently transport patient with
the chart to the ward as per patients convenience and endorse patient to
Ward nurse
5. Emergency Room Nurse fills the admission logbook and enters data in the
bizbox data base.
For MINOR SURGICAL CASES Procedure

Step 1

Patients for minor surgical procedure must proceed to the Triage Area for
evaluation, assessment and registration, or to the treatment area for emergency
management.

Step 2

The patient's significant other should claim the Patient Data Registration
Form from the Triage Nurse before proceeding to the Admitting Section for the
processing of the admission (choosing of room, signing of Patient's Data Sheet,
filling up of HMO Agreement Form or making a partial payment.

Step 3

Go back to the Emergency Room and proceeds for minor surgical procedure

1. Emergency Room nurse have Patient sign consent


2. Emergency Room Nurse prepares the needed equipment and supplies for the
procedure.
3. Once procedure starts, Emergency Room nurse attends to the needs of the
physician, E.R nurse then records name of procedure, type of anesthesia,
time started and time ended, name of Physician and supplies used.
4. After procedure ended and resident physician makes home medication orders
patients chart is then sent back to OPD DEPARTMENT
