Dear Respondents,

Greetings! We, the members of Group 5 of BSN 3A are currently doing a study in partial fulfilment
of the requirements for the degree of Bachelor of Science in Nursing with our study entitled The
relationship of the 3 psychological needs, Self Esteem and alcoholism among the students of Velez
College. This study aims to know which of the 3 Psychological Needs; namely (1) autonomy, (2)
relatedness and (3) competence greatly contributes to self-esteem and the effects of self-esteem to the
alcoholism among Velez College students. You had been selected as one of the 312 respondents to
partake this study as you have met the following criteria: (1) Velez College student officially enrolled
for summer semester of S.Y. 2017 and (2) 18-25 years old. To fulfil this endeavour, we would like to
ask a few minutes of your time to answer these three sets of questionnaire:

The first questionnaire is a 21 item form which will inquire regarding how well each psychological
needs is generally satisfied the daily life. It is answered through a 7-point Likert scale ranging from 1
for not true at all to 7 for very true.

It is then followed by a 10 item questionnaire that will ask your subjective emotional evaluation of
self. In answering this you will have to choose if you Strongly agree, Agree, Disagree or
Strongly disagree to the following statements.

Lastly, the third questionnaire is a 24-item yes or no test which will revolve around alcohol-
related problems and practices.

The questionnaire would only take about fifteen minutes to accomplish and the data gathering would
only be done at one point. We guarantee that all information and answers you supply will be treated
with utmost discretion. Your name will not be asked in the questionnaire and a control number had
been printed in each questionnaire for tallying proposes. Your questionnaires will be kept in a box
with a lock with the key only accessible to the researchers. The return of this questionnaire would
imply your willingness to be a part of our study. Light snacks will follow after answering the
questionnaires. Thank you for your patience in reading this letter. We hope for a favourable return.
