My Patient Decription

On last semester, I have a task to assess a patient condition. I have a patient named

Mr.Aryo. He is 35 years old and now he lives in Kudus City. He has a small family which lives

happily. He has 2 daughters and wife. His occupation now is a teacher at junior high school.

Mr.Aryo is a kind, sociable, wise and religious person. He always helping other people

who need help from him. He also has many friends that come from his office, and neighbours.

Everytime, he always dhikr no matter where he is. He told me that he felt more peace if he doing

that up all day.

Mr. Aryo has good physical appearance. His height is about 170 cm, and his weight

about 68 kg. He has an oval face, brown narrow eyes, sharp nouse, and white skin. He also has

black wavy hair. His cheek is thin, no pimple or scar on his skin. He does not like to smoking, so

his teeth is clean and white.

Mr.Aryo complains on his General Practicioner about his condition. He feels dizzy,

decreased appetite, weak and he told that more than a week pass without defecation, and it

difficult or even painful to pass. He said that he has lumpy and hard stools. He is feeling as

though there is a blockage in his rectum that prevents bowel movements.

Mr. Aryo feels so weak, and he need long rest. For a two days, he must absent from his

job. Everyday, he should accompanied her daughter to go to school, but now, he can not do that

as usually. Her wife told me that one week ago Mr.Aryo is very busy and has a lot of activities to

do. Mr. Aryo does not keep his healthy, he does not ever drink plain water. He also rare to do

simple sport, like joging, or stretching.

Mr. Aryo also complain about pain sensation on his rectum. After defecation, he feels so

painful. I asked abou how many scale of that pain, and he answered the scale is about 7. I know,

he has constipation for along time, and the stools absolutely hard. But, the pain will be disappear

for several minutes.

Everytimes he feels not comfort with his stomach, and this condition makes his job as a

teacher does not work well. He told that he does not confidence with his big stomach. He does

not like to eat some vegetable or fruit, and this makes his conditon getting worst. The General

Practicioner examined him, and the GP said that the blood presure of Mr.Aryo is low. It is ninety

over sixty miimeters of mercury. Besides, the body temperature is still normal, it is 36,5C. His

respiratory rate is 24 rpm, and his pulse is 80 bpm. His doctor give him medicine named opilax. It

helps for relieve constipation. I suggest Mr.Aryo to eat more fiber in vegetable and fruit such as

papaya, banana, and spinach, and drink more plain water for at least 8 glasses a day.
