1. Respectfully request conducts ballistic examination on the accompanying specimen to

determine the firearm described below was fired from the cartridge case or otherwise.

A. NATURE OF CASE : Violation of Omnibus Election Code Illegal Possesion of

Firearms- Imporvised Firearms SUMPAK

B. SUSPECT : RIZALDY CATAHAN Y MORALES, 45 years old, male,

unemployed and presently at Purok2 Barangay Mojon, Malolos
City, Bulacan

C. TDPO : May 6, 2016 OOA 03;30PM in Barangay Mojon, Malolos City,

Bulacan.

D. Specimen Submitted :

SPECIMEN A - one (1) improvised firearms (sumpak) and marked as MCM

SPECIMEN B - one (1) live ammunition (12-gauge) with marking as MCM-1

2. Further request that this Office be furnished a copy of ballistic examination result for our
reference.
