I, __________________, hereby declare under penalty of perjury pursuant to 28 U.S.C.

1746 that the following is true and correct.

1. a. I have been a hourly paid, non-exempt police officer with the Puerto Rico Police

Department from _______ to _______.

2. I worked at the following precinct(s) as a police officer:

3. I regularly was not paid for all time worked as a police officer.

4. I regularly arrived to my precinct and performed work prior to my scheduled shift. I

observed other police officers in similar situations. My supervisors also observed myself and

other police officers performing work prior to our shift. I was not paid for this time.

5. I regularly was required to give testimony in Court. I sometimes had to do so on my days

off. If I was paid for this time, I was not paid for all time, or an overtime rate of one and a half

times my regular hourly rate.

6. I incurred expenses for my boots and my uniform. I have not completely or properly been

reimbursed for these expenses.

7. During the time I was a police officer, there has never been a poster informing police

officers that they had a right to be paid for all work performed and to be paid time and a half for

all time in excess of 8 hours a day or 40 hours a week.

8. This Declaration is written in English and has been translated to Spanish for my benefit.
