1.1 General

The AMT 2500 PLUS laser systems is specially designed to minimize

accidental exposure to hazardous radiation.

The areas of concern for safe CO2laser operation are discussed in this

chapter.

1.2 Burn Hazard

CO2 laser radiation is invisible to the human eye and can cause third

degree burns, even when unfocussed.

1.3 Reflection and Direct Eye Exposure Hazard

The system output beam visible and invisible laser radiation that is

hazardous to the eye.

Never stare into the CO2 laser beam or allow it to be reflected from any

reflective surface-even rough metal can reflect the CO2laser beam. And

never stare into the aiming beam directly. Although this beam is low

powered, direct exposure can be hazardous to eyes.


1.4 Safety Eyewear

All personnel in the vicinity of the laser unit must use safety eyewear-all

patients must wear the safety eyewear during doctor's operation.

And make sure that the eyewear provides adequate protection from the

10.6micron wavelength radiation.

This is generally provided by most quality safety glass spectacles with

side guards for protection from lateral exposure. The American Optical

spectacle-type safety glasses, or equivalent, provide ample protection.

Safety eyewear is not required when viewing through a microscope,

colposcope or endoscope, as the glass lenses of these instruments

provide sufficient protection.

1.5 Patient Safety

For enhanced patient safety, perform the following safety procedures:

1. Surround the surgical area with wet towels.

2. Make sure that safety eyewear is available to the patient, if conscious.

2. For facial surgery, completely cover the patients eyes with

moistened eye pads.


1.6 Explosion and fire Hazard

Do not operate the unit in the presence of volatile substances(or

flammable anesthetics) such as alcohol, gasoline or solvents.

1.7 High Voltage Hazard

The AMT 2500 PLUS laser system generates high voltages within the

main case. To avoid injury, do not attempt to remove or disassemble any

panels. Only AMT engineering authorized technical person may service

the unit.

1.8 Using the Proper Power Receptacle and Plug

Use only the power receptacle and plug specified for your unit.

Use only a power receptacle and plug that are in good condition

Use only a hospital grade plug and a correctly matched power receptacle.

To remove the power cord from the receptacle, hold it by the plug.

Never pull on the power cord to remove the plug from the

receptacle.
1.9 Grounding the Unit

The unit is grounded through the grounding conductor in the power

cord. Good grounding is essential for safe operation. To ensure

grounding reliability, always plug the power cord into a properly

wired hospital grade power receptacle. Additional grounding can be

provided by using the external ground connection point(see2.4).

1.10 Fuse Replacement

The user may replace a line fuse that is located beside the power

cord connection on the auxiliary panel. To avoid fire hazard, use only

the fuse specified for your unit.

Replacement fuse must be identical in type, voltage rating and current

rating to the original fuse. (Warning : Before removing a fuse, turn

off the key switch, remove the power cord from the ac outlet.).

1.11 Compliance with international Standard

The AMT 2500 PLUS laser system complies with:

U.S. Federal Performance Standards 21 CFR 1040.10 and 21 CFR

1040.11 for Class laser products European Directive 93/42/EEC

Concerning Electromagnetic Compatibility.


The AMT 2500 PLUS laser system is designed to comply with the

IEC601-1 Standard for safe Use of Electro medical Equipment, the IEC

825-1 Safety of Laser Products - Equipment Classification Requirement,

and the User's Guide for Class Laser Products.

In compliance with these standards, the AMT 2500 PLUS laser system

incorporates an emission indicator, a CO2 laser beam shutter, a power

display, a master key switch and proper labeling.

The system also incorporates an input power transformer that ensures a

leakage current of less than 150 .

In accordance with these regulations, a recommended routine inspection

and maintenance schedule is provided in the maintenance chapter of this

manual.
1.12 Warning, Identification and Certification Labels

The important labels affixed to the unit. These include:

1. Laser emission danger label - warning against possible exposure to

laser radiation and specifying the types of laser present.

2. Non - interlocked warning label-warning against.

a. Risk of explosion if used in the presence of volatile substances.

b. Possible electrical shock when cover is removed.

c. Possible radiation exposure.

2.1 Unpacking and Inspection

The AMT 2500 PLUS laser systems have passed full quality assurance

testing before shipment. Thus, the unit should be operational upon

delivery. The unit should be unpacked, installed and tested only by an

authorized AMT engineering representative. Do not attempt to unpack or

assemble the unit.


2.2 Equipment List

The AMT 2500 PLUS laser systems includes the following:

2.3 Line Power Considerations

The unit is delivered factory - set for the proper ac line voltage rating of

the country to which it is shipped. The ac line voltage appear on the label

affixed to the rear panel.


2.4 Foot switch Connection

The foot switch is in the yellow box. To connect the foot switch to the

unit, position it on the floor and plug its cable into the FOOT SWITCH

connection point on the rear panel. A polarizer on the socket prevents

incorrect connection of the cable.

2.5 Articulate Arm

The articulate arm is in the yellow box. To assemble the articulate arm

to the unit, position it in socket on the unit and screw the socket nut of

articulate arm. For good assembling, look at the red aiming beam. If the

red aiming beam is seemed to be focused, then its alignment is to be

done.

3.1 General Laser Theory

LASER is an acronym for Light Amplification by Stimulated Emission of

Radiation. The laser is a device consisting of an active medium enclosed

in an optical cavity, and a pumping source. The pumping source "pump"

the active medium from its ground energy state to its excited energy

states. if population inversion between two excited states takes place

(where the higher energy state is more populated), stimulated emission

of radiation (photons) can occur, This radiation is bounced back and forth

in the optical cavity, and is amplified. This amplified electromagnetic

radiation is emitted as a laser beam.

The properties of the beam are:

1. High degree of collimation - unidirectional beam with a very small

divergence

2. Monochromaticity - the radiation is withen an extremely narrow

wavelength range on the spectrum.

3. Coherence - all photons are in phase, both in space and time.


The active medium of a laser can be either gas, liquid or solid. Most gas

lasers consist of atoms, small molecules, or mixtures of both. Solid state

laser consist of atoms of ions doped in some solid matrix. Liquid lasers

consist of higher molecular weight molecules dissolved in liquid.

Under specific pumping conditions, all these materials can undergo the

unnatural phenomenon of population inversion, which results in

stimulated emission of radiation at a wavelength characteristic of the

active medium.

The beam's transversal power distribution of mode structure determines

its quality. For medical applications, the best choice is the TEMoo mode,

wherein the beam has a Gaussian (bell-like) transversal power

distribution, ensuring a minimal spot size, and thus, maximal power

density on tissue.

3.2 CO2 Laser Theory

The AMT-2500 PLUS laser system incorporates a sealed- off CO2

laser tube, The active medium used is a proprietary gas mixture. The gas

mixture is enclosed within a glass laser tube equipped with electrodes.

The laser tube is positioned between two mirrors of the optical cavity.
The rear mirror is totally reflective, whereas the front mirror is partially

reflective and transmits the CO2 laser beam into the unit's articulate arm.

The dc power supply(pumping source) provides voltage to

the electrodes which produce an electrical discharge along the tube.

The discharged electrons collide with the CO2 molecules in the gas

mixture and excite them to a vibrationally excited level(an asymmetric

stretching mode). The stimulated emission takes place between this level

and a lower vibrationally excited level(a symmetric stretching mode),

resulting in laser emission in the far0 infrared range at a wavelength of

10.6 microns.

3.3 General System Description

The AMT-2500 PLUS laser systems is an advanced microprocessor

controlled, user friendly CO2 laser systems based on a sealed-off CO2

laser tube providing up to 25 watts on tissue. The system incorporates

the CO2 laser tube within the main cabinet, an articulated arm beam

delivery system and attachable laser accessories. The unit is activated

for laser emission by a foot switch.


3.4 Main Cabinet

The main cabinet includes the following;

3.4.1 Poser Supply

The AMT-2500 PLUS laser systems are equipped with a switching-

mode power supply which converts the input line voltage to the high

voltage required for laser operation.

The inherent advantages of the switching-mode power supply over the

conventional power supply are its small physical dimensions, high energy

conversion efficiency and increased safety from high voltage hazards,

wherein the high voltage components are activated only during laser

emission.

3.4.2 Laser Cooling System

The laser tube cooling system is a closed-loop pressurized system. The

coolant is circulated by the pump.


3.4.3 Control Panel

The microprocessor-based soft-touch control panel is used to control

all operating functions. It includes a built-in status display, which

provides direct on-line monitoring of all functions for safe system

operation.

3.4.4 Air Flow System

The air flow system consists of an air compressor, a medical

grade sterilizable silicone tube that extends along the articulated arm.

3.4.5 Optical bench assembly

The optical bench assembly is comprised of the sealed-off laser tube

and its optical resonator, the CO2 laser shutter assembly, the diode

aiming beam intensity control assembly, and the CO2 and aiming beam

combiner.

The beam combiner combines the CO2 laser beam and aiming

beam coaxially, and guides then into the articulated arm beam delivery

system.
3.5 Articulated Arm Beam Delivery System

The beam delivery system is a lightweight, spring-balanced 7joint

articulated arm. It includes two arms and a 3-knuckle endjoint.

The AMT-2500 PLUS laser systems articulated arm beam delivery

system provides excellent maneuverability, adequate working distance

and full wrist motion.

3.6 System Modes of Operation

The AMT-2500 PLUS laser systems modes of operation are based on

two distinct types of settings: the laser operation mode and the tissue

exposure mode settings. The laser operation modes control the power

pattern and the peak power outputs, while the tissue exposure modes

control the duration pattern of the laser beam delivered to tissue.

The system offers three operation modes and three tissue exposure

modes as follows:

1. C.W (Continuous Wave) : power range control

2. ULTRA (Ultra pulse) : Frequency range control

3. PW (Power Wave) : Pulse width control

Note. The TEMoo mode structure (bell-shaped power distribution) is

preserved in all modes of operation


3.6.1 Basic Concepts and Definitions

1. Frequency (f) - number of pulses per second.

2. Period or cycle time(T) - the time interval between the onset of two

consecutive pulse. (T = 1/f: T is inversely proportional to f.)

3. ON Time and OFF time - the time duration during which the laser is

operative within the cycle time(T) is called the ON time, whereas the

time duration during which the laser is inoperative within the cycle

time(T) is called the OFF time. Thus,

Cycle time(T) = ON time + OFF time

4. Duty cycle(%) - the ratio between the ON time and total cycle time(t),

given as percentage:

Duty cycle(%) = ON time 100/T

5. Peak power(Ppeak) - the maximum power delivered by the laser

beam during emission.

6. Average power (Pav) the time averaged power.

7. Pulse width(w) - the pulse duration at half peak power level.


Note - In this section, "ON time", "OFF time", and "duty cycle" refer

only to the laser operation mode, and not to the tissue exposure mode.

The peak power and average power are relation in the following manner:

a. For a square wave periodic function

(such as in the CONTINUOUS WAVE REPEAT mode):

Thus, to obtain a variation in average power at a constant peak power,

the duty cycle is adjusted accordingly.

b. For a general periodic function(such as in the SUPER PULSE laser

operation mode), a good approximation of this equation of these

parameters,

Thus, the average power can be controlled either by varying the peak

power, the pulse width, the frequency or any combination of these

parameters.

In the SUPER PULSE laser operation mode, the AMT-2500

PLUS provides the highest possible peak power to obtain the

requested average poser.

In the PULSE laser operation mode, the system provides a constant

peak power for a selected average power. This average power is

obtained by varying the ON time and maintaining the frequency constant.

3.6.2 Laser Operation Modes

The laser operation modes are CONTINUOUS WAVE(CW), SUPER

PULSE and PULSE.


In the CW laser operation mode, the laser beam is emitted in

continuous wave, whereas in the SUPER PULSE and PULSE laser

operation modes the laser beam is emitted as a train of very narrow

pulses.

In the CW laser operation mode, a continuous wave laser beam is

emitted int the power range of 1 - 25W.

In the SUPER PULSE laser operation mode, the laser beam is emitted in

a train of very narrow high-peak-power pulses. The SUPER PULSE

frequency and the ON time are adjusted by the microprocessor to obtain

the desired average power by maintaining either the ON time or the

frequency constant. The power display shows the SUPER PULSE

average power, which ranger between 0.1 - 7. 0 W.

In the PULSE laser operation mode. the laser emits high-frequency

pulses in a constant preset pulse frequency. The average power

requested and delivered to tissue is controlled by the microprocessor by

varying the ON time(the duty cycle). The power display in this mode

ranges between 1 - 25W

Tissue incision capability is generally enhanced with higher peak

power of the laser beam. Thermal damage to surrounding tissue is


usually reduced with shorter laser activation durations, whereby the

adjacent healthy tissue has more time to cool between pulses.

The SUPER PULSE and PULSE laser operation modes enable these

tissue effects to be achieved through their constant high peak power

level and variation of the duty cycle for control of average power.

Figure 3-2 illustrates the different power versus time patterns

generated for each of the laser operation modes. Note that although the

average power can be identical in all three laser operation modes, the

peak power, ON time and OFF time differ. The treated tissue reflects

these differences in terms of incision/ablation effects and of thermal

condition to adjacent tissue.

operation mode

3.6.3 Tissue Exposure Modes

The tissue exposure modes control the duration pattern of the laser

beam delivered to tissue. The three tissue exposure modes are

CONTINUOUS, SINGLE PULSE and REPEAT PULSE. The power versus

time pattern of each tissue exposure mode in the three laser operation

modes is illustrated in Figures 3-3 through 3-5.


In the CONTINUOUS tissue exposure mode, the laser beam is emitted for

as long as the foot switch is pressed.

In the SINGLE PULSE tissue exposure mode, the laser beam is emitted

as a single pulse(for the C.W laser operation mode) or as a single

burst(for the SUPER PULSE and PULSE laser operation modes), for a

preset duration(ON time), or until the foot switch is released, witchever

comes first. For C.W laser operation mode, the ON time range is 0.01 -

1.0 sec. For SUPER PULSE laser operation mode, the ON time range is

0.06 - 1.0sec. For PULSE laser operation mode, the ON time range is

0.16 - 1.0sec.

In the REPEAT PULSE tissue exposure mode, the laser beam is

delivered intermittently as a series of bursts for a preset duration,

determined by the ON time and OFF time values, for as long as the foot

switch is pressed. The selected OFF time (emission duration) determines

the interval between bursts, while the selected ON time determines the

number of burst.
Note

In Figure 3-3 through 3-5, "ON time" and "OFF time" refer only to the

tissue exposure mode.

4.1 General

The following is a description of the AMT-2500 PLUS laser systems

controls, indicators and connection points according to location.

4.2 Control Panel

The control panel is designed to provide simple and comprehensive

control of the system through soft-touch controls and displays. The LED

indicator illuminates when the key function is in operation. Positive key

contact is indicated by a brief beep.

A. Mode Key : Selection key (CW, ULTRA,PW)

select Less or More key for energy.

C. Frequency key : After press down Freqkey, select Less or

Morekey for Frequency.

D. Pulse width key : After press down Pulse widthkey, select Less or

Morekey for pulse width.

E. Memory & Save key : After you adjust proper energy and frequency

etc and you want to save it for a long time, press down Savekey. Then

the data will be saved in the memory chip automatically. You can save 3

data. If you wish to save over 3 data, press down Savekey and at the

same time the oldest data will be erased.


F. Up & Down key : Adjustment key for Power, Freq and Pulse

width.

G. STBY / READY key : Before you want to choose Mode, Power,

Freq and Pulse width, you have to press down Stbykey in advance.

After then, you press down Readykey, and you will see the Ready light

is simmering. Then press down Foot switch.

6.1 Introduction

This chapter contains the AMT-2500 PLUS laser systems routine

inspection and maintenance instructions. Only those procedures

designated herein may be carried out by clinic/hospital staff. All other

procedures designated herein may be performed only by AMT

engineering - authorized technical personnel.

6.2 Service Information


The AMT-2500 PLUS laser system generates high voltages within the

main cabinet. Therefore, the interior of the unit should be checked by

AMT engineering - authorized technical personnel only.

All correspondence with your AMT representative, regarding the unit,

should include the model and serial numbers appearing on the

identification label.

6.3 Routine Maintenance

The unit should be periodically inspected and serviced to maintain it in

optimum condition. A recommended routine inspection and maintenance

schedule is given in Table 6-1.

6.4 Fuse Replacement

To check and/or replace the fuses on the auxiliary panel, turn off the unit

and disconnect

Warning

Before removing a fuse, turn off the key switch, remove the power cable from

the power receptacle and wait approximately two minutes to allow for high

voltage discharge.

Caution

For continuous protection against fire hazard, replace fuses only with the same type

and rating of fuse. Fuse type is slow-blo 3A/250V or equivalent.

For exact fuse rating, refer to the fuse listing on the rear panel.
6.5 External Cleaning / Disinfection

The external surfaces of the unit and the foot switch should be

disinfected when the unit is initially received, and thereafter as required

by clinic/hospital protocol. The surfaces may be wiped with a damp cloth

of cotton dipped in a hospital grade 70% alcohol solution of a disinfectant

liquid.

Caution

1. Remove the power cord from the ac outlet before cleaning/ disinfecting with liquids.

2.Only the external surfaces of the unit may e cleaned by clinic/hospital staff.

For care and cleaning of the laser surgical accessories, refer to Chapter 6

6.6 Handpiece sterilization

The handpiece lens and Mirrors may be cleaned by clinic/hospital staff,

following the procedures.

1) Remove the handpiece from the articulated arm.

2) Remove the lens cell from the handpiece.

DO NOT STERILIZE THE LENS CELL.

3) Detach the purge gas hose from the handpiece horse barb
4) Detach the handpiece tip from the handpiece.

5) The Handpiece body and the handpiece tip may be sterilized by

autoclaving or by using ethylene oxide gas.

6) While holding the sterilized handpiece body with sterile gloves, a

circulating nurse can push the unsteriled lens cell into the handpiece

body, taking care not to contaminate the handpiece body

7) Reattach the handpiece tip to the handpiece body using sterile gloves.

8) If purge gas required, the circulating nurse should push the gas purge

hose onto the handpiece hose barb. taking care not to contaminate the

handpiece

6.7 Lens and Mirror Cleaning

The lenses and mirrors within the unit may be cleaned only by AMT

engineering - authorized technical personnel

The handpiece Lens and Mirrors may be cleaned by clinic/hospital staff,

following the procedures in chapter6

6.8 Power Calibration Check

The calibration of the unit power meter should be checked to need

Calibration check. However, only AMT engineering - authorized

Technical personnel may perform the actual calibration procedure

1. Attach a handpiece with a 100mm lens to the endjoint, and position

the external power meter 20mm beyond the focal point of the beam.

Turn on the key switch and aim the Laser diode beam at the power meter.

2. Select CW laser operation mode and set power to 25W

3. Press ENTER to select external lasing

4. press the foot switch

5. Record the power level reading that appears on the external

power meter

6. Release the foot switch

7. Repeat steps 1 through 6 for 25, 20, 15, 5, 2, and 1W At

each power level, compare the message display reading to that of the

external power meter. If the deviation between the two readings is more

than 10% perform the power meter calibration procedure

7.1 General

Should the AMT engineering laser unit malfunction, consult the

Troubleshooting Guide in this chapter to identify the possible cause.

Clinic/hospital staff may perform the troubleshooting procedures in this

chapter, except where specifically stated that troubleshooting must be

carried out by AMT engineering - authorized technical personnel only.

Warning
This unit generates high voltages and emits high-intensity laser radition. Improper
handling of the unit can cause serious injury

Caution

Improper use of adjustment of this device may invalidate the AMT engineering

Service Warranty Agreement. Contact your AMT engineering representative before

any attempt to troubleshoot this unit in any manner other than as specified in this

manual.
7.2 Troubleshooting Guide

Table 7-1 is a troubleshooting guide for fault status which may appear

on the message display and can be corrected by the clinic/hospital staff.

Table 7-2 lists some possible system symptoms indicating malfunctions

that do not appear on the message display

If corrective action, listed in both tables, does not solve the problem,

contact your authorized AMT engineering service representative, The

troubleshooting tables do not attempt to anticipate all possible failures.

Any fault not listed in the table must be referred to AMT engineering -

authorized technical personnel.
