menurut peraturan-peraturan di dalamnya, bersama dengan segala dividen, deposit atau tambahan dalam sebarang bentuk, jika ada. Tanggungan
Takaful Ikhlas Berhad ke atas atau berkaitan dengan Sijil tersebut mulai tarikh ini adalah ditetapkan dan terhad hanya kepada baki yang ada di dalam
akaun pelaburan peserta, serta lebihan, jika ada, dan selepas membayar pengeluaran ini, tanggungan Syarikat tersebut terhadap pengeluaran
sebahagian nilai akaun pelaburan peserta akan dan dengan ini dilepaskan.

Setiap orang yang melaksanakan perjanjian ini mengesahkan bahawa pada masa ini, Sijil ini tidak diserah hak, melainkan seperti yang ditunjukkan di
bawah oleh tandatangan penerima serah hak / pemegang (pemegang-pemegang amanah, jika ada, dan bahawa tiada prosiding kebangkrapan telah
diambil oleh atau terhadapnya atau mereka.

I, the participant of the above Takaful Certificate hereby apply to withdraw the amount stated above from my certificate for its value in personal
investment account according to its terms, together with all dividend, deposits or addition of any nature if any. The liability of Takaful Ikhlas Berhad upon
or in connection with said certificate is as of this date fixed and limited to the remaining balance in personal investment account, and credits, if any, and
upon payment of the request thereof, the said Company shall be and is hereby discharged.

Each person executing this agreement certifies that this certificate is not now assigned, except as indicated below by the signature of the assignee /
trustee (s), if any, and that no proceedings in bankruptcy have been instituted by or against him, her or them.
