Go to Url http://jindsrv0203/MICAHome/ then click Applications.

Go to JGC Indonesia -> Human Resource -> Medical Reimbursement Claim and then click
Claimed.

2. INPUT MEDICAL REIMBURSEMENT CLAIM

First select Patient medical reimbursement claim, if only one it will be automatically selected.

Second click button Add Detail to input detail medical reimbursement claim.

Third input detail that consist of Bill No, Bill Issued By (Hospital name or Clinic name), Bill Date,
Cost Type, Cost Detail, Diagnose, and Claim Amount. Click button OK after finish input
detail. You can add more than one detail by click button Add Detail again.

Fourth scan all hardcopy in single pdf file and attach to Collective Attachment, right now this
attachment still optional.

Fifth you can know your plafond (maksimum you can claim), approved (claim that already
approved), on process (claim that you create but not yet approved), and balance (remaining
amount you can claim) from your medical reimbursement claim.

Sixth input Comment and select Action after that click button Execute. Action Submit request
will go to HRD and Action Save as Draft request will go to your task to do in Url
http://jindsrv0203/MICAHome/ to complete later. After you click button Execute there will be a
confirmation if you click Yes claim will process, if you click No will be back to claim form.

3. TRACKING MEDICAL REIMBURSEMENT CLAIM

Go to Url http://jindsrv0203/MICAHome/ then click Applications.

Go to JGC Indonesia -> Human Resource -> Medical Reimbursement Claim and then click
Claim Tracking.

Input Process ID to search specific Claim ID in this manual example 162. Then click button Search.
From result list click link View to view details and click link Print to print document if you forget print
after submit.
