LBP 20 to compression of L2 (L), HTN stage II.

Present problem list:

1. Chronic intermittent localized dull aching pain on (B) knees (L>R), (PS on knee
8/10), (PS on (L) knee 10/10) elicited by weight bearing and is relieved by rest.
2. Decreased muscle strength of (L) hip flexors and (L) knee extensors graded 4/5
3. Grade 1 pitting edema
4. Crepitation of (B) knees upon knee extension

Present PTMx:

1. HMP c TENS on (B) knees x 20


2. US on (B) knees x 1.5 w/cm2 x 5
3. Wall slide c VMO setting using white ball x 5SH x 10 reps x 1 set
4. LE ergo bike x 5
5. Quadriceps setting x 7SH x 10 reps x 1 set
6. Isomets on (B) glutes x 7SH x 10 reps x 1 set

Px was seen and treated 2x this month of September 2016.

Check pxs skin before, during and after application of HMP. Assist px in sit to stand.

1. Chronic dull aching pain on (B) knees (PS 7/10) and on (B) paralumbar (PS 8/10)
elicited during sit to stand and standing activities an is relieved by rest.
2. Good standing balance and fair standing tolerance
3. Moderate difficulty in ambulation and sit to stand requiring +1 assist (contact
guarding)
4. Muscle weakness of (B) UE graded 4/5
5. Postural deviation (kyphotic posture, genu varum)

1. PREs on (B) UE and LE using 2lbs DB and MAW x 10 reps x 1 set
2. HMP on (B) paralumbars and knees x 20

5. Gait ambulation (as tolerated) (MWF)

1. Generalized muscle weakness on (B) shoulder flexors 3-/5, elbow flexors, wrist
flexors and knee extensors graded 3+/5
2. Poor standing tolerance and fair standing balance
3. Decreased grip strength on hand in comparison to the (L) hand

1. PREs on (B) UE and LE using 1lb DB and LE MAW towards AP x 10 reps x 1 set

Px was seen and treated 15x this month of September 2016.
Px is easily distracted and is uncooperative. Px also insists on walking during //
bar exercise.

3. Postural deviation (forward head and rounded shoulder)
4. Gait deviation (stride length, step length, cadence)

Monitor pxs BP during exercise. Let px rest before proceeding to the next exercise .
