EVENT ANANLYSIS REPORT

2. Operating Conditions at the time of Event:
Load:168 Coal Cycles In service: B,C,D,E,F Oil support:NIL

3. Nature of Event: Set tripped as SSB Transformer-II tripped due
to malfunctioning of Winding Temperature.

4. Name of First Up, Main Protections & protection on which
GCB tripped: 1)Turbine trip ckt faulty.2)Turbine trip on
protection.3 )Turbine trip solenoid energised.4)Low fwd. power
interlock.5)Master trip Relay 86 GT operated.6)Service Aux. Xmer
tripped on Fault. 7) SAX2 transformer wdg.Temp Tripped

5A. Observations : T.A Set sharing 168 MW Load With 5 mills in
service, due to failure of SSB transformer II ,Incomer II on
emergency board tripped & APH 6A,LOP & FD FAN 6B Tripped, Boiler
& turbine tripped at 5:05 Hrs.

5B. Remedial Action : 1) Indicators calibration and contacts
actuation was checked and it was found healthy.2) Cable from
indicator to breaker (6.6 kV) was found healthy.

6. Root Cause Analysis : SSB Transformer II tripped at 05.05 hrs
on dt 19.12.2014. Following alarms appeared in control room:1)
Winding temperature high alarm and trip appeared
simultaneously.2) Service Auxiliary Transformer tripped on fault.
Winding temperature protection has been provided through a
digital indicator with contacts and sensing element RTD pt100.
Contact for high & very high are configured for alarm(at 80 deg
C) and trip(at 100 deg C) The temperature of winding was 37 deg
C at the time of tripping. The indicator was immediately checked
and the same was found healthy. The cable from indicator to HT
breaker was also checked with the help of megger. The cable is
also healthy. No fault was detected in the circuit, the
activities to synchronize the set were started. it was decided
to keep the same indicator in service with alarm in service and
protection of Very high temperature bypassed to observe the
recurrence of the same fault.

7. Preventive Action Suggested Short Term: It was decided to keep

be replaced shortly by new one in the next available opportunity

12. Remarks:
1)Testing Section:-a) SS tr. No.2 tripped on wdg. Temp. high
protection and hence SSB no. 2 tripped. All the compressors on
SSB 2 tripped. SSB charged through tie of SSB no.4. and
compressors started.b) SS TR. No. 2, Winding temperature alarm
and trip ckt. Checked found OK.Suspected Wdg. temp digital
indicator malfunction .c) After meggering, again SStr.2 charged
and SSB no. 2 again taken in service. (wdg. Temp. trip bypass and
alarm kept in service.)Half emergency board charged with bus
coupler from emergency transformer side.d) 50 V DC battery
charger restored.e) IAC and SAC restored. f) Bag Filter PLC was
hanged. Reseted it by removing damper and gate connections.g)
Field breaker trip coil replaced and control ckt work
attended.h)Generator field breaker on /off ckt checked and trial
taken.i)Light up and synchronizing activities attended.
2)EM Section:-a) Reported SSB Xmer no. 2 tripped. Hence station
service board load shifted to SSB Xmer no. 4. Then SSB Xmer no. 2
checked its IR values found normal. Hence station service board
charged again through SSB Xmer no. 2. And bus coupler made OFF.
Permit cancelled.
3) BM Section:-a) C/M 6B PM of mill done b) C/M 6F Worm
shaft gland leakage from DE and NDE attained. Spring gap
adjusted. All oil guns cleaning done. CV3 valve made operative.
