EVENTS
0730 hrs: Vessel stopped her main
engines when she was 6 cables from
No.1 berth and doing speed of 7.3
knots

0732 hrs: Vessel started swinging to


Stbd to head towards No.1 berth.
Distance was 4 cables and speed 5.6
knots

0734 hrs: Vessel stopped swinging to


stbd and commenced swinging to port
to clear No.1 berth. Distance was 2
cables and speed 4.3 kts

0736 hrs: To increase rate of turn to port


so as to clear no.1 jetty edge, bow
thruster was given full to port and
ahead movement dead slow & then
slow ahead was given.

0737 hrs: Full astern was given as


bridge team realized she would not
swing clear of No.1 berth.

0737~0738 hrs: Vessel made contact


with No.1 berth, head on, breaching
her hull in way of Forepeak tank

Do not get distracted by computers, mobile phones or
gossips. Be Alert and have situational awareness
