Correlation Between Platelet-to-White Blood Cell Ratio (PWR) and Clinical Severity

Based on NIHSS in Acute Ischemic Stroke Patients

Background: Stroke is a major cause of mortality, with the Indonesia Basic Health Research
2007 showed the prevalance of stroke was 8.2/1000 people and 67.1% were diagnosed
ischemic stroke accounted for the majority of cases. Previous studies showed there was
positive independent correlation between platelet to white blood cell ratio (PWR) and
stroke severity existing. Therefore, the objective of this study was to analyze the correlation
between PWR and clinical severity based on the National Institutes of Health Stroke Scale
(NIHSS) in acute ischemic stroke patients.

Methods: This retrospective cross-sectional study using total sampling involved 71 patients
diagnosed with acute ischemic stroke at the Department of Neurology, Dr.Hasan Sadikin
Hospital, between February and March 2017. Data were collected using medical records and
analyzed using Spearman R correlation with p < 0.05.

Results: In this Study containing 71 patients with age (mean 55,9), sex (35,2% man, 64,8%
woman). Regarding parameters, trombocyte (mean 298373,2 mg/l, SD 109708,1), leukocyte
(mean 10736,6 mg/dl, SD 3583,7), PWR (mean 30,0 SD 12,8), NIHSS on admission (mean
10,7 SD 5,8). There was significant correlation between PWR and NIHSS on admission (R=
-0.27 and p value : 0,0222).

Conclusion: There was Correlation Between PWR and clinical severity based on NIHSS.
The decreased of PWR is significantly increased NIHSS, so the patients were worse and
which could be used to know prognostic indicator in acute ischemic stroke cases.
