hereinafter referred to as the Company, after having been sworn in accordance with law, hereby
depose and declare that;

1. Section 16 of the 1987 Philippine Constitution provides that, the State shall protect and
advance the right of the people to a balanced and healthful ecology in accord with the
rhythm and harmony of nature;

2. Executive Order No. 26 s. of 2011, otherwise known as the National Greening Program
(NGP) was issued by His Excellency, President Benigno Simeon Aquino III, having a goal
for biodiversity conservation, climate change adaptation and in addition efforts of poverty
reduction and food security by implementing sustainable reforestation projects that will
include enhance livelihood, environmental protection, education and public health;

3. Executive Order No. 193 s. of 2015 or Expanding the Coverage of the National Greening
Program (NGP), involved the participation and investment of the private sector with a view
towards enabling private companies to achieve carbon neutrality;

4. Under the Implementing Rules and Guideline of EO No. 193 s. of 2015, private sectors can
develop and invest in areas, either tenured or untenured for the establishment of new NGP
plantations;

5. The Company is steadfast in contributing to the betterment of the society and cooperating
to the priority programs of the government on poverty reduction, resource conservation
and protection and climate change mitigation;

6. Pursuant to the preceding, the Company will adopt an area of ________ hectares located
at __________________________________ to be planted with forest trees and ornamental
plants, hereinafter referred to as the Tree Growing Program (TGP);

7. The Company undertakes to:

a. Identify the area for the TGP with a minimum area of ________ hectares;
b. Provide ________ seedlings of ________or other indigenous tree species that are
suitable in the subject area;
c. Plant the said seedlings in the area identified;
d. Protect and maintain the identified area ;
e. Provide information to the City Environment and Natural Resources Office (C-ENRO)
of the _____________ whenever there are any unfavorable incidents/events that may
hamper the objective of this agreement ; and
f. Submit quarterly monitoring report to C-ENRO on the development of the adopted
area.
