DEED OF WAIVER OF RIGHTS

Know all men by these presents

That this Waiver of Rights is being executed this ___ day of _____2017 in _______________, by and
between:

We, RODOLFO O. TAMBAGAN, BRONWYN O. TAMBAGAN, BRIDGITTE T. JAVIER,

BEVERLY O. TAMBAGAN, MONICA BERYL T. GO, ROBERT O. TAMBAGAN, LUCIA BREANNA T.
WILLIAMS, and ROLANDO O. TAMBAGAN, of legal ages, Filipino, and residents of 955 R. Higgins Brgy.
185, Gate 3, Pasay City, Philippines, after being sworn in accordance with law, hereby declare that:

1. That we are the surviving heirs CARIDAD ORCALES TAMBAGAN who died on 06 June
2017 at San Juan De Dios Educational Foundation Inc. (Hospital) located at 2772 Roxas
Blvd., Pasay City;

2. During her lifetime she maintained an amount of Two Hundred Fourteen Thousand Three
Hundred Forty One and 11/100 centavos (Php.214, 341.11) with the Air Materiel Wing
Savings and Loan Association, Incorporated (AMWSLAI) under account no.002-10-01-
019818-6;

3. That we, RODOLFO O. TAMBAGAN, BRONWYN O. TAMBAGAN, BRIDGITTE T. JAVIER,

BEVERLY O. TAMBAGAN and ROLANDO O. TAMBAGAN, the undersigned, do hereby
waive, transfer and convey all our rights, interest and obligations over said AMWSLAI
account left by our above deceased mother;

4. That this waiver of rights is being executed in favor of our sisters, namely: LUCIA BREANNA
T. WILLIAMS and , who affixed their signature herein below as manifested and their
acceptance of the same and hereby assumes any obligations that may arise with the said
claim;

IN WITNESS WHEREOF, we have hereunto set our hands this 12th day of July, 2017 in
