III. PROBLEM PRESENTED:

Julieta is asking for financial/ medical assistance in behalf of her daughter, client Mark
Jean, 17 years old. The client was diagnosed by her attending physician to have caesarian
operation. Presently, the family expressed their adversity in raising the needed amount
worth Php58, 944.98 for the said surgery, thus asking for government assistance.

IV. FAMILY BACKGROUND AND SOCIO-ECONOMIC STATUS:

Julietas daughter in law is 17 years old. The girl can be described with a good looking
and intelligent. She was graduated as a valedictorian when she was in high school. She got
pregnant at the age of 16. Her partner doesnt have a stable job, and works as a laborer earning
an estimated monthly income pf Php1, 500 in irregular basis.
The couple were currently staying at the girls parent for they are not yet ready to live on
their own. Her mother is a plain housewife while her father is a regular government employee
at City Mayors Office (CMO) with a monthly income of Php11, 000.
The family compound is located at Purok 1, Barangay Batomelong, General Santos City.
They owned the lot and house which made up of light materials with concrete floor.
V. ASSESSMENT STATEMENT:
Julietas daughter in law is in need of financial/ medical assistance to pay their
remaining balance worth Php58, 944, 98 at Doctors Hospital.

VI. RECOMMENDATION:
Based on the assessment and data gathered by the undersigned, the client is
therefore recommended to avail financial/ medical assistance from the Congressional
Office through Congressional Pedro Acharon.
