APPENDIX F

REQUIREMENTS ON DESIGN REVIEW AND INSPECTION SERVICES

1. DESIGN REVIEWS

1.1. Material Capacity

The Consultant shall consider applying a material capacity reduction factor for
used material for his design reviews for Temporary Works (including falsework
and support system for superstructure). The material capacity reduction factor
shall be applied on actual capacity (test results on new samples).

1.2. Checks

The Consultant shall carry out design reviews on the formwork structure design
in accordance with Workplace, Safety and Health (WSH) Regulations using a
Factor of Safety (FoS) of 2.0. Upon completion of his reviews, he shall advise
LTA in writing on adequacy of the design submitted by Contractors PE.

2. INSPECTIONS

2.1. Checklist

a) The Supervising Qualified Person, QP(S), shall develop and adopt a standard
inspection checklist (as shown in Attachment 1 and to be used as a guide)
which is to be used by formwork supervisor, contractor & himself.

b) He shall conduct his inspection at the site and ensure that all checks are
carried out to cover material quality, verticality, dimensions, base support,
bracing, connections, access, lighting, formwork joints, etc, by the respective
formwork supervisor and contractor before signing off the inspection checklist.

c) All works shall not proceed until the standard inspection checklist has been
duly checked and signed off (any actions required, if any, are to be carried out
and fully rectified) by all parties concerned.

2.2. By Professional Engineer (PE)

a) The Supervising Qualified Person, QP(S) , and his Supervision Team shall
ensure that the Contractors PE inspect and certify the formwork structure
twice, prior to the installation of reinforcements and 24 hrs prior to
concreting as required in the WSH(Construction) Regulations 63(5) and
65(3).

b) The Supervising Qualified Person, QP(S), and his Supervision Team shall
ensure that where more than one PE is involved in the checking of the
Temporary Works (including falsework and support system for
superstructure), each PE should carry out these inspections and issue their
respective Certificates of Supervision.

3. CONCRETING

a) The Supervising Qualified Person, QP(S), and his Supervision Team shall
ensure that no person is allowed into or under formwork structure after
300mm depth of concrete has been poured.

b) The Supervising Qualified Person, QP(S), and his Supervision Team shall
also note that only Formwork Supervisor shall enter below formwork
structure for monitoring, such as for leaks. There shall be no more than 2
workers to assist in any repairs (if required).

c) The Supervising Qualified Person, QP(S) and/or his Supervision Team shall
note that concreting shall stop for any formwork repair.

d) There shall be no entry and repairs allowed after 300mm thickness of


concrete is poured. QP(S) and his Supervision Team shall ensure that the
completed area is to be barricaded off.

e) The Supervising Qualified Person, QP(S) and/or his Supervision Team shall
note that any remedial works at later stages shall be assessed by
PE/Contractor/QP(S)/LTA.

f) The Consultant, his Supervising Qualified Person, QP(S), and Supervision


Team shall familiarise himself and comply with the Workflow for Formwork
Structure Inspection Process as shown in Attachment 2.
