JUDICIAL AFFIDAVIT

Purpose of the testimony: Herein plaintiff is presenting the testimony

of PO1 JOSE REYES to prove that the incident occurred on July 24, 2017;
that defendant driver Emmanuel Nicassio was negligent in driving GB II Bus
with plate no. AKG 994 to which the plaintiff was boarded. That
immediately after the incident defendant Nicassop immediately tried to
escape but was apprehended by PO1 JOSE REYES.

I, PO1 Jose F. Reyes, of legal age, Filipino citizen, with postal address
at #2154 Matimayas street, Lealtad Sampaloc City, after having been sworn
in accordance with law and fully conscious that I do so under oath and that I
may face criminal liability for false testimony or perjury in a way of answer
to the questions propounded to me by ATTY. Norfatimah Sunggod, with
office address at 1344 Taft Avenue, Ermita, Manila.

ATTY. Sunggod: We are offering the testimony of the witness in order

to prove that he is the investigating officer at the time and place of the
incident;
Q1: Mr. Witness, can you state your name and designation?

A: I am PO1 Jose Reyes, a Police Officer of Manila City Police

Department

Q2: can you recall what your task was at the night of July 24, 2017?

A: I was tasked to come with PO2 Aquino to do some patrol

around the area of Sampaloc Manila.

Q3: Can you recall what happened during your patrol at about
10:00 pm?

A: Maam we were already along Espaa Boulevard when a Bus

suddenly swerved to the right and thereby sideswiping another
Bus.

Q4: Can you identify the Bus that suddenly swerved?

A: yes Maam, it was a GB Bus with plate no. AKG 994.

Q5: what did you do after that?

A: I immediately told PO 2 Aquino to stop the car and pull over.

Q6: After that? What did you do?

A: I immediately got out of our car to examine the two Busses.

Q7: Mr. Witness, can you state for the record what happened after
that?

A: I saw the defendant driver coming out from the window and
immediately ran, I chased him and was able to caught him, I
placed him at the backseat of our patrol car and An Ambulance
came and assisted those persons who were injured due to the
incident.
