** La Ansiedad y Depresin son unos de los factores psicolgicos de riesgo ms importantes en los trastornos
del sueo (hasta un 63 % de pacientes con ansiedad presentan insomnio y hasta un 90% de los pacientes
con depresin presentan algn tipo de trastorno del sueo).
Crisis de angustia: aparicin sbita de sntomas de aprensin, miedo pavoroso o terror, acom-
paados de sensacin de muerte inminente y sntomas somticos como sensacin de ahogo,
palpitaciones, opresin torcica, sensacin de atragantamiento y miedo a volverse loco o per-
der el control.
Agorafobia: aparicin de ansiedad o comportamientos de evitacin en lugares o situaciones
donde escapar puede resultar difcil o embarazoso o bien donde sea imposible encontrar ayu-
da en caso de que aparezca una crisis de ansiedad.
Fobia especfica: presencia de ansiedad clnicamente significativa como respuesta a la expo-
sicin a situaciones o objetos temidos, lo que suele dar lugar a conductas de evitacin.
Fobia social: presencia de ansiedad clnicamente significativa como respuesta a ciertas situa-
ciones sociales o actuaciones en pblico.
Trastorno obsesivo-compulsivo: se caracteriza por obsesiones que causan ansiedad y males-
tar significativo y/o compulsiones cuyo propsito es neutralizar dicha ansiedad.
Trastorno por estrs postraumtico: reexperimentacin de acontecimientos altamente traum-
ticos, sntomas debidos al aumento del arousal y comportamientos de evitacin de los est-
mulos relacionados con el trauma.
Trastorno de ansiedad generalizada: presencia de ansiedad y preocupaciones de carcter exce-
sivo y persistente durante al menos 6 meses.

Nota: Como mnimo, uno de los sntomas marcados con asterisco (*) debe estar presente para poder reali-
zar un diagnostico de Trastorno Depresivo Mayor. Asimismo, debe haber una presencia de al menos cinco
sntomas de los sealados durante un periodo igual o mayor a dos semanas para un diagnstico de Trastor-
no Depresivo Mayor.
Aminas terciarias
Amitriptilina 25-125 mg Con la Amitriptilina se tiene amplia experiencia
en dolor neuroptico
Imipramina 25-125 mg Producen un bloqueo intenso de los receptores colinr-
gicos, histamnicos y alfa-adrenrgicos, responsable de
sus intensos efectos secundarios
Iniciar con 12,5 mg por la noche e incrementar
dosis cada 3 das, hasta 25-50 mg y observar efectos
Aminas secundarias
Notriptilina 25-125 mg Poseen un menor efecto anticolinrgico
La Nortriptilina produce, tambin, menor bloqueo
adrenrgico
Desipramina 25-125 mg Iniciar con 12,5 mg por la noche e incrementar dosis
cada 3 das, hasta 25-50 mg y observar efectos
IRSS (Inhibidores de la recaptacin de la Serotonina)
Citalopram 20-60 mg En personas mayores no sobrepasar los 40 mg/d
Fluoxetina 10-40 mg Tiene un potente metabolito activo (norfluoxetina) con
una V 1/2 de eliminacin prxima a las 240 h
Iniciar, en pacientes ancianos, con 5 mg. (se dispone
en solucin)
Administrar en dosis nocturna
Fluvoxamina 50-300 mg Tiene efecto sedante. Administrar por la noche
Paroxetina 10-40 mg Es el que produce una inhibicin ms potente del cito-
cromo P450, con riesgo elevado de interacciones
medicamentosas
Administrar por la maana en una sola dosis
Sertralina 50-200 mg Es el que produce una menor inhibicin del citocromo P450
Iniciar con 25 mg y escalar dosis en 3 das
Administrar por la maana o noche
Duales (Inhibidores Recaptacin Serotonina-Noradrenalina)
Velanfaxina 37,5-150 mg Debe supervisarse estrechamente su administra-cin en
pacientes con antecedentes de cardiopata isqumica,
convulsiones y glaucoma
Mirtazapina 15-45 mg La Venlafaxina puede producir hipertensin arterial,
por lo que debe vigilarse este parmetro
Psicoestimulantes
Metilfenidato 5-30 mg No administrar en pacientes con ansiedad asociada
Se recomienda no administrar despus de las 14 h por
el riego de insomnio
Puede producir o empeorar la anorexia
Iniciar: 5-10 mg desayuno y 5 mg comida y aumentar
progresivamente segn respuesta
