Creating the Philippine Council for Mental Health

The Philippine Constitution provides that the State shall protect and promote the

right to health of the people, instill health consciousness among them and adopt

an integrated and comprehensive approach to health development which shall

endeavor to make essential goods, health and other social services available to all

the people at affordable cost

The council shall undertake the following:

Formulate policies and guidelines on mental health issues and concerns;

Develop a comprehensive and integrated national plan on mental health;

Conduct periodic monitoring, evaluation and research in support to policy

formulation and plan on mental health;

Promote and facilitate collaboration among sectors and disciplines for the

design, development and implementation of sectoral Mental Health plans;

Review all existing laws related to mental health and recommend legislations

which will sustain and strengthen existing programs and services and other

mental health initiatives;

Advise the President in the field of mental health; and

Submit annual report to the President.

Philippine Mental Health Act of 2015

The Philippine Mental Health bill recognizes the need to protect the rights and

freedoms of persons with mental health needs and to reduce the burden brought

by their condition.

The bill aims to forward an integrated, well-planned, effectively organized and

efficiently delivered mental health-care system that responds to their mental health

needs in equity with their physical health needs.

4) Senate Bill 2910

An act promulgating a National Mental Health policy towards the enhancement of

integrated mental health services, the promotion and protection of persons utilizing

mental health services, approaching the funds therefor and for other purposes

5) Natasha Goulbourn Foundation

Jeannie Goulborn is a renowned Philippine fashion and interior designer. She

developed Manilas ready-to-wear fashion for ladies. She was part of the Philippine

Tatlers list of Whos Who of the Philippines.

She is an advocate for promoting an enhanced understanding about depression.

This was brought about by the passing of one of her daughters due to depression. With

the help of friends, she established Natasha Goulbourn Foundation, an organization

that would promote awareness about depression and help people suffering from it.

Knowing about depression can decrease the risk of suicide

The Natasha Goulbourn Foundation (NGF) started embarking in its mission to

promote awareness and understanding for Depression as a medical condition when it

was founded in 2007. Through a series of local and international lectures of forums,

aided with media mileage on TV, print and radio, people have started to become aware

of what depression is all about, and how at its worst, it can lead to suicide.

NGF saw the need to deliver the information and education drive at a much faster

and wider scale. In this, two programs were developed. The first being the development

of a curriculum for the INFORMATION AND CRISIS INTERVENTION CENTER (ICI).

The curriculum was co-authored by Dr. Cornello Banaag, Dr. Ma. Lourdes A.

Carandang, Dr. Eleanor Ronquillo and Dr. Ricardo Soler who are leading mental health

professionals. Information and Crisis Intervention Centers are centers of

communication where one-to-one telephone exchanges assist and support callers in

crisis due to depression or suicide issues and/or callers who are not necessarily in

immediate crisis but may need help to prevent a crisis from occurring or to

descalate an ongoing one.

The second is the continued observance of WORLD SUICIDE PREVENTION

DAY. First initiated by the International Association for Suicide Prevention and with the

participation of the World Health Organization, World Suicide Prevention Day is

observed every September 10 as a call to support programs for suicide prevention,

remember lost loved ones and support survivors of suicide.

Vision

The Natasha Goulborn Foundation shall work to promote a healthier society

of Filipinos through the use of positive and preventive activities that will focus on

MENTAL WELLNESS FOR ALL. We believe that it only though strengthening the

physical, emotional, mental and spiritual aspect of an individual that true happiness

and wellness thrive.

Mission

o To promote an exemplary mind and body health care, especially addressing

the issues of depression including to the extent possible, its management

and/or treatment; and

o To bring together support groups, organization and institutions to make

different modalities of mental health care services accessible to the

disadvantaged/marginalized groups in the society.

Objectives

o Education and information campaign on depression targeting students, the

Academe, mental health care providers, Overseas Filipino Workers (OFWs)

and their families in the community level.

o Set up a resource center housing a library, workshop and consultancy

rooms.

o Network and forge partnerships with like-minded local and global individuals

and organizations involved in the integrative programs and services within

the reach of the local communities.

o Create programs that will train suicide responders to man crisis hotlines

which will eventually be taught to all teachers and guidance counselors so

that depression among the youth is seen early on and treated immediately.

6) Hopeline Project

Hopeline Project aims to strengthen its campaign to prevent the

occurrence of suicides in the country. Hopeline is a phone-based counseling

service available 24/7 to individuals who suffer from crisis situations and

depression.

The launching of the project was in collaboration with Natasha Goulbourn

Foundation (NGF) and Globe Communications.

The project will give emphasis on three Cs Connect, Communicate and

Care which is also the theme for the 2016 World Suicide Prevention Day. The

numbers to call are the following: (02) 805-HOPE (4673); 0917-558-HOPE (4673)

and 2919.

Hopeline representatives who answer the calls can explain to them that

suicide is not an option and there are ways to deal with it, like talking to a doctor

who can manage the condition.

7) National Suicide Prevention Week

National Suicide Prevention Week (NSPW) is an annual week-long

campaign in the United States to inform and engage health professionals and the

general public about suicide prevention and warning signs of suicide. By drawing

attention to the problem of suicide in the United States, the campaign also strives

to reduce the stigma surrounding the topic, as well as encourage the pursuit of

mental health assistance and support people who have attempted suicide.

According to Major David Reynolds, chief of Clinical Health Psychology at

Landstuhl Regional Medical Center, The vast majority of people dont commit

suicide because they want to kill themselves, but a way to end the torment of not

being able to cope with their problem. Suicide rates for young adults ages 15-24

rose 136 percent from 1960 to 1980. Suicide remains the third leading cause of

death among people between the ages of 12-24.

National Suicide Prevention Week is the Sunday through Saturday

surrounding World Suicide Prevention Day, September 10.

8) Why do we need a mental health law?

Every Filipino should be entitled to mental health programs and services.

Mental health problems in the country has long been dismissed and not given any

priority at all.

We are facing a global human rights emergency in mental health. All over

the world people with mental disabilities experience a wide range of human rights

violations.

In 2014, the report Health for the Worlds Adolescents by the World Health

Organization (WHO) revealed that depression is the predominant cause of illness

and disability for both boys and girls aged 10 to 19 years old. Unipolar depressive

disorder is one of the projected leading causes of disease and injury burden by

2030, according to the 2004 WHO Global Burden of Disease Report. And since

the burden of depression and other mental health conditions are seen to be on the

rise globally, the World Health Assembly, in a resolution issued in May 2012, called

for a comprehensive, coordinated response to mental disorders from health and

social sectors at the country level.

A research conducted by Lapena, Tarroja, Tirazona & Fernando of DLSU-

SDRC on the Filipino youths views on mental health revealed that the problems

confronting the youth include lack of resources and conflict within themselves or

with others. The researchers recognized that some of the youths coping strategies

are positive seeking help, actively resolving, and talking about the problem. But

they also noted how other strategies that the youth use such as engaging in

negative and escapist behaviors, excessive drinking, taking drugs, acting out, or

simply ignoring the problem are not desirable. This research showed that even

without intervention, Filipino youth are able to cope with their difficulties by relying

on their inner resources.

In order to build a mindstrong nation and be armed with socio-economically

and psychologically competent citizens, we must first acknowledge the fact that

there are no programs and policies designed to protect the mentally ill, prevent

mental illness, and promote mental health in the country at present. As they say,

its better late than never.

To invest in mental health is to invest in national development. To raise

mindstrong Filipinos is to raise the nations competency and resilience while

fostering hope and optimism and strengthening the holistic wellness of the

Filipinos.

9) Enumerate and explain the six principles of Effective Mental Health Program

by Anselmo Tronco

Address stigma. People with mental health illness confront two challenges

the syndrome of the sickness itself and the discrimination they experience from

the people around them. According to WHO, stigma has negative effects to the

patients as it deteriorates self-esteem, disrupts family relationships and limits

patients ability to socialize and secure housing and jobs. People thinks

mentally ill patients are violent and dangerous but in fact patients have higher

tendency to hurt themselves than hurting other people. Dr. Tronco urgerd for

mental health awareness and campaigns be included in mental health

programs.

Experience a process of sharing, validation, and problem-solving. Build

support groups where communities can share experience of difficulties, identify

common reactions, validate good stories of coping, and facilitate plans for

action. Aside from the family where patients can gather strength and resilience,

community is a single most important resource for Mental Health interventions.

Create relationship with the stakeholders for mental health. Local chief

executives have important role in integrating a mental health program in the

community because they have the power to bring together services from the

health and non-health services.

Involve the family in the program. As partners in providing care to persons

with mental disorder, families also need some form of support. Family go

through immense pressure in taking care of a patient, thats why, services such

as family education and training, support, and counseling on mental health

should be available and accessible to them. Like support groups for the

patients, support group where family members can get opportunity to share

fears and learn lessons on coping should be established.

Harness Community Resilience on Mental Health. Build on the successes

of the mental health programs in the communities. Look at the social and

cultural practices on how people in communities overcome stress and deal with

the challenges of life. Harnessing community resilience also requires human

resource that can effectively mobilize community services and infrastructure for

mental health.

Grab opportunities from crisis and disasters. When disaster strikes, leaders

usually listen and responds to communal pain. There are opportunities for

mental health programs to highlight emotional and social needs since disasters

creates a context of shared grief and recovery.

10) Enumerate and explain Mental Health Care Law: Ten Basic Principles

Promotion of Mental Health and Prevention of Mental Disorders. Everyone

should benefit from the best possible measures to promote their mental well-

being and to prevent mental disorders.

o Promoting behaviors which contribute to enhancing and maintaining well-

being, such as those identified by WHO;

o Identifying and taking appropriate actions to eliminate the causes of mental

disorders, such as those identified by WHO.

Access to Basic Mental Health Care. Everyone in need should have access

to basic mental health care.

o Mental health care should be of adequate quality

o Access to mental health care should be affordable and equitable

o Mental health care should be geographically accessible

o Mental health care should be available on a voluntary basis, as health care

in general

o Access to health care, including mental health care, is contingent upon the

available human and logistical resources

Mental Health Assessments in Accordance with International Accepted

Principles. Mental health assessments should be made in accordance with

internationally accepted medical principles.

o Mental health assessments include: diagnosis, choice of a treatment,

determination of competence, determination that someone may cause harm

to self or others due to a mental disorder

o Mental health assessments should only be conducted for purposes directly

relating to mental illness or the consequences of mental illness.

Provision of the Least Restrictive Type of Mental Health Care. Persons

with mental health disorders should be provided with health care which is the

least restrictive.

Self-Determination. Consent is required before any type of interference with

a person can occur.

Right to be Assisted in the Exercise of Self-Determination. In case a patient

merely experiences difficulties in appreciating the implications of a decision

although unable to decide, he/she shall benefit from the assistance of a

knowledgeable third party of his or her choice.

Availability of Review Procedure. There should be a review procedure

available for any decision made by official (judge) or surrogate (representative)

decision-makers and by health care providers.

Automatic Periodical Review Mechanism. In the case of a decision affecting

integrity (treatment) and/or liberty (hospitalization) with a long-lasting impact,

there should be an automatic periodical review mechanism.

Qualified Decision-Maker. Decision-makers acting in official capacity or

surrogate shall be qualified to do so. To be qualified, decision-makers should

be: competent, knowledgeable, independent, and impartial.

Respect of the Rule of Law. Decisions should be made in keeping with the

body of law in force in the jurisdiction involved and not on another basis nor on

an arbitrary basis.

11) Enumerate and explain the General Principles of Mental Health Nursing.

Patient is accepted exactly as he is. Acceptance conveys the feeling of being

loved & cared. Acceptance does not mean complete permissiveness but setting

of positive behaviors to convey to him the respect as an individual human

being.

Use self-understanding as a therapeutic tool. Psychiatric nurses should

have a realistic self-concept & should be able to recognize ones own feelings.

Her ability to be aware & to accept her own strengths & limitations should help

her to see the strengths & limitations in other people.

Consistency is used to contribute to patients security. There should be

consistency in the attitude of staff, ward routine & in defining the limitation

placed on the patient.

Reassurance should be given in a subtle & acceptable manner. To give

reassurance, the nurse need to understand & analyze the situation as to how

it appears to the patients.

Patient behavior is changed through emotional experience & not by

rational interpretation. Major focus in psychiatry is on feelings & not on the

intellectual aspects. Advising or rationalizing with patients is not effecting in

changing behavior.

Unnecessary increase in patients anxiety should be avoided. The

following approaches may increase the patients anxiety and should therefore

be avoided:

o Showing nurses own anxiety

o Showing attention to the persons deficits

o Making the patient to face repeated failures

o Placing demands on patient which he cannot meet

o Direct contradiction on psychotic ideas

Objective observation of patient to understand his behavior. It is the ability

to evaluate exactly what the patient want to say & not mix up ones own

feelings, opinion or judgment.

Maintain realistic NPR. This focuses the personal emotional need of the

patients & not on the patients needs.

Avoid physical & verbal forces as much as possible. All methods of

punishment should be avoided.

Nursing care is centered on the patient as a person & not on the control

of symptoms. Analysis and study of the symptoms is necessary to reveal their

meaning and their significance to the patient.

All explanation of the procedure & other routines is given according to

the patients level of understanding. The extent if explanation that is given

to a patient depends on his attention span, level of anxiety & level of ability to

decide.

Many procedures are modified, but basic principles remain unaltered. In

psychiatry, many procedures are modified but the underlying nursing principles

remain the same.

1. Did you learn anything from the videos?

Yes. Ive come to realize and accept that these kind of things do really happen in

reality. Being sad and sometimes down is not that bad, as long as you will eventually

stand up on your own and learn to reach out to other people, who might be there for

you.

2. What are the messages of the stories?

Ive always viewed people who had suicide to be weak. But after watching the

videos, Ive came to realize that it is not their fault nor what they want. Its the world

and the people who made them retort to that kind of decision. We may think that they

are weak for they do not want to face the problems they are facing, but may be in fact

they are brave enough to come up to that kind of decision.

3. Did anything happened in the videos remind you of something that has occurred

in your own life that you have seen occurred to others, too?

Nothing. But when stressed out, I become depressed and feel like ending

everything as well. Because thats what the society does to people, it makes you think

you are not enough to live on. But come the next day, with the sun rising above your

head, youll open your eyes to the reality that you are not the only person having it

hard, you might be having it easier than most population.

4. What were you thinking as you finished the videos?

Every time I watch these kind of videos/stories, I come to realize how lucky I am

to be able to walk the streets, greet people with a smile, and not to worry where Ill be

getting the money for the next day. I realized how lucky I am to have such precious

family and friends who are just there for me when I need someone to confide to.

5. Would you recommend these videos to a friend? Why?

Yes. It is unfortunate that these events happened to them but it will be a great

learning for those who are still breathing. It might be the answer to all their dilemmas,

or at least help them be inspired and be motivated to live on.

6. What life lessons can be learned from the videos?

Words. So powerful. They can crush a heart, or heal it. They can shame a soul,

or liberate it. They can shatter dreams, or energize them. They can obstruct connection

or invite it. They can create defenses, or melt them. We have to use words wisely. (J.

Always be kind to anyone, use kind words and give them a good smile. You dont

know what is inside that persons head, you dont know what shes going through, you

dont know her. You dont know how a simple smile can actually lift her mood up, and

motivate her to live the day.

7. Few months from now, you will be leaving the portals of your Alma Mater. As a

true Perpetualite and a BSN graduate, how can you address the present mental

health issues and concerns in your own community?

You cant change what you refuse to confront.

We cannot deny the fact the mental health issues are evident and everywhere. We

must not turn a blind-eye and wait for them to do something that will one day haunt us

down.

For starters, I will be an advocate of community mental health. Use my voice and

represent those who cannot speak for themselves.

As a future nurse, I will not be judgmental and be a good listener. I will set myself

aside and put someones wellbeing before me.
