Diesel Generator Damage

2. Outline of breakdown
During voyage with two Turbo Generators and one Diesel Generator operating in parallel, the said
generator suddenly stopped. Cooling water was observed leaking from the vicinity of the individual
cylinder covers.
Investigation revealed that the suction valve rod for No. 8 cylinder in bank A had broken in the middle and
had dropped inside cylinder. The cylinder cover and piston head were damaged. The connecting rod was
bent. Lower parts of cylinder liner and piston were also broken.
Besides, it was observed that Nos. 1-3 cylinders in bank A were broken. Lower parts of Nos. 2 and 3
cylinders pistons were damaged. No. 2 and 6 suction valve seats were struck. Connecting rods of Nos.1 to
4 and No. 6 cylinders were bent.

(*) No abnormality was found in crank shaft in maker's inspection.

4. Probable Cause
Due to clogging or oil shortage in locker arm oil passage for the No. 8 cylinder, suction valve seizure
occurred and stuck at an open position. The suction valve rod broke and dropped into the combustion
chamber, and was struck by piston, causing damage to cylinder cover (forming a hole), and cooling water
entered the combustion chamber through the hole.
This gave rise to liquid compression, which bent the connecting rod and broke the lower part of the piston
and liner.
In addition, cooling water that entered through the suction manifold caused damage to other cylinders.
