Discussion of completed service learning project:

For my service learning project I attended American Diabetes Association health fair at the Honda
Plant in San Antonio, TX. According to City of San Antonio Metropolitan Health District
(SAMHD) (2013), more than 1 in 7 San Antonio (Bexar county) residents have been diagnosed
with Diabetes type II, and 16% of people in Bexar county have Diabetes type II compared to 10%
in the state of Texas, and 9% in the entire United States (SAMHD, 2013). I assessed the life style,
medicaition regimen, and family history of Honda employees with diabetes and pre-diabetes. In
addition, I provided teaching when it was necessary.

Evaluation of the selected intervention:

Skin integrity: teach the patient to care for his or her feet, not to go outside without shoes, advise to
see a Podiatrist regularly especially if patients begin to experience neuropathy. Dominic et al.
(2015) stressed that nurses are responsible in educating patients and family the disease
management, and are crucial in prevention of neuropathic ulcer (p. 299). Goals of diabetic teaching
on skin integrity were met because Honda employees returned teaching on skin integrity and the
need for protective foot wear; also, restated the outcome of compromised skin integrity and
acquired diabetic foot ulcers; in addition, employees discussed the need of seeing a Podiatrist
regularly.

Discuss how this project contributed to professional collaboration & community engagement:

Part of the professional nursing is concentrated on community health nursing, which focuses on
health promotion and disease prevention of a community, and teaching clients the importance of
such things. The Essential VII in the American Association of Colleges of Nursing (AACN)
(2008) emphasize that Health promotion, disease, and injury prevention across the lifespan are
essential elements of baccalaureate nursing practice at the individual and population levels. These
concepts are necessary to improve population health (AACN, 2008, p. 23). By participating in
community health events such as American Diabetes Association health fair, I had an opportunity
to perform a role of a community health nurse. All of the Honda employees who participated in
the health fair were treated as a client.
Nurses in a professional role collaborate with other professionals to identify community needs and

prioritize interventions keeping in mind the priority of health promotion and disease prevention.
Honda employees with pre-diabetes received teaching on prevention of diabetes; employees with
diabetes received teaching on importance of skin integrity and prevention of diabetic foot ulcers.
The representative from American Diabetes Association (ADA) was well prepared with
information and handed out flyers and booklets on diabetes management and prevention including
recepies and food choices, and importance of glucose checks for clients with a family history of
diabetes. My collaboration as the nurse with a social worker from ADA improved the outcome for
diabetes prevention in pre-diabetic clients, and customized a care plan for diabetic clients based on
clients goals.
