PROMISSORY NOTE
FOR THE VALUE RECEIVED, I promise to pay Bukidnon State University Non-Teaching Employees
Association the sum of TWENTY SIX THOUSAND SIX HUNDRED PESOS (P 26,600.00) at the rate of 24% per
annum payable in 12 MONTHS installment.

I also hereby authorize the TREASURER of Bukidnon State University Non-Teaching Employees
Association to collect from the payroll master of Bukidnon State University the sum of ONE THOUSAND
PESOS PER MONTH until this loan is fully paid.

In case of any default in payment as herein agreed or should by employment with BUKIDNON
STATE UNIVERSITY be terminated, the entire loan balance shall immediately be due and payable without
need of notice or demand.
