CHANGING A URINARY DIVERSION OSTOMY APPLIANCE

Definition: Urinary diversion is a surgical procedure that reroutes the normal


flow of urine out of the body when urine flow is blocked.

Purpose: To minimize odors for the clients comfort & self-esteem

RATIONALE:
1. Obtain data

14. (FROM A TO C) - toilet tissue, used gently will not damage the stoma. The gauze absorbs
any drainage from the stoma while the skin is being prepared. Cleaning the skin removes
excretions and old adhesives and skin protectant. Excretions or a build-up of other
substances can irritate & damage the skin.

15. (FROM A TO C) - The appliance should fit snugly around the stoma, with only 1/8” of
skin visible around the opening. A faceplate opening that is too small can cause trauma to
the stoma.
