CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT made and entered into this 17th day of January 2018
in the Municipality of Calape, Province of Bohol by and between:

BOHOL ISLAND STATE UNIVERSITY represented herein by DIOSCORO A. AVERGONZADO,

PhD, OIC President of Bohol Island State University, with postal address of CPG Avenue, Tagbilaran
City, Bohol, hereinafter referred to as UNIVERSITY:

-And-

The Group of Arbiters represented by MR. THADDEUS M. SIMPAO with postal address in
Tagbilaran City, Bohol herein referred to as OFFICIATING OFFICIALS of the Soccer and Futsal
Tournaments for Men and Women;

WITNESSETH, That:

Whereas, the Bohol Island State University desires to hire the services of the association to
officiate during the Soccer and Futsal Tournaments of the Region 7 SCUAA Meet 2017;

NOW THEREFORE, both parties hereby mutually agree to the following stipulation, to wit:

1. That Bohol Football Association will prepare the rules and regulations and officiate the
Soccer and Futsal Tournaments both in Men and Women’s Division during the Region 7
SCUAA Meet 2017 meet.
 2. That the Officiating Officials will be paid Twenty Six Thousand Pesos (PhP 26,000.00) as
an honorarium for the services rendered.

3. That payment shall be released after the event, winners are declared and the supporting
documents (e.g. result of the games) will be submitted.

IN WITNESS HEREOF, we have hereunto affixed our signatures this 17th day of January 2018
at Bohol Island State University-Calape Campus Calape, Bohol, Philippines.
