JUDICIAL AFFIDAVIT

I, Melissa Narciso y Fernandez, 33 years old, single, direct seller,

Filipino and a resident of Phase 8-A, Block 191 Lot 1, Phase 11 Bagong
Silang, Caloocan City, plaintiff in this case state under oath as follows:

PRELIMINARY STATEMENT

The person examining me is Fiscal Ethel Kathleen Tugade, a

Public Prosecutor. I am answering her questions fully conscious that I
do so under oath and may face criminal liability for false testimony and
perjury

QUESTIONS AND ANSWERS

1. Q: Do you swear to tell the truth, the whole truth, and nothing but
the truth in this investigation?
A: Yes, Ma’am.

2. Q: Do you understand that the testimony you will give today is
under oath; you may face criminal liability for false testimony or
perjury?
A: Yes, Ma’am.

3. Q: What happened last September 20, 2012 at more or less 9

o’clock in the evening?
A: Yes, ma’am. I was at home cooking for dinner, when I saw my
daughter returned from playing with her friends outside. She
approached me and said that Regina Ignas Y Hizon said something
to her.

4. Q: What did she say?

A: My daughter told me her exact words were, “Hay naku hawag
na lang, andiyan ang anak ng kabit.(Nevermind, the mistress’ child
is here)” My daughter was confused and asked me what did Regina
mean.

5. Q: What happened next?

A: I was gravely hurt and offended with what she said to my
daughter.

6. Q: What did you do next?

A: I told Benjamin Ignas, the father of my child, that his wife said
those words towards my daughter. He said that he will go to the
house of Regina Hizon to talk to her.

7. Q: Then what did you do next?

A: It was around 11 p.m. I went to the house of my niece Mary Joy
Pedrero y Ignas. While I was chatting with Nanay Conching, I saw
Regina entering the house along with her children Lovelyn, Reiben
Alea Mei, Tyan ng and Marjorie.

8. Q: What happened next?

A: She approached me and said why I had to tell the incident to
Benjamin. She suddenly pulled my hair, scratched my face and with
her children kicked me hard.

 A: I managed to escape away from them and decided to go to the
Task Force of Barangay Public Safety Department Office (BPSDO).
I met policeman Roberto Alfaro y Jimenez and filed a barangay
blotter against Regina and Lovelyn.

10. Q: What did you do next?

A: The following day I went to the Pres. Diosdado Macapagal
Memorial Medical Center because I had injuries on my face.

11. Q: What happen during the medical check-up?

A: I was examined by the Doctor. The Doctor said I had multiple
abrasion facial area, sub conjunctival and hemorrhage of the right
eye.

12. Q: Do you have medical certificate to prove that you went to the
hospital for medical treatment?
A: Yes, mam. I have a medico-legal certificate.

13. Q: Do you intend that it form part of your judicial affidavit?

A: Yes, mam.

14. Q: Do you have something more to say, add, or change to your

answers in this affidavit?
A: No more, mam.
