The Board of Directors (the "Board") of Willow Fork Drainage District (the
"Distrrcl"), met in regular session, open to the public, on April 29, 2009, at the Willow
Fork Country Club, 21055 Westheimer Parkway, Katy, Texas, inside the boundaries of
the District, and the roll was called of the members of the Board:
Also present at the meeting were Dennis Gorden of Severn Trent Services 1"5T");
Fran Matuska of F. Matuska,Inc.; Melony Gay of Uretek/ICR; Lou Triche and Rob Karl
of Champions Hydro-Lawn, Inc.; Larry Mueller of AECOM, Inc.; and Stephen M.
Robinson and Patti Porter Hopper of Allen Boone Humphries Robinson LLP ("ABHR").
APPROVE MINUTES
The Board first reviewed the minutes of the March 12, 2009, meeting. After
review and discussion, Director Robinson moved to approve the minutes as submitted'
Director Renberg seconded the motion, which carried unanimously.
There were no residents present wishing to address the Board. The Board
discussed a report from a District resident regarding a neighbor driving a four-wheel
vehicle along the ditches and illegal dumping of yard debris. The Board requested Mr.
Triche to drive by the site and take pictures.
SECURITY SERVICES
Director Lyons reported on security matters. The Board requested Ms. Matuska
to request Deputy Stark to provide the report with the payroll information.
Mr. Triche stated one of the deputies requested the Board to authorize placing a
sign on the channel behind the South Wastewater Treatment Plant. The Board
concurred to place a sign at the location.

MAINTENANCE OF STORM SEWER LINES
Mr. Gorden distributed the storm sewer maintenance report. Ms. Guy
introduced herself to the Board and reviewed the types of services provided by Uretek
in sealing, Iifttng, and stabilizing the ditch outfalls. In response to a question from
Director Ward, Ms. Gay stated the product is environmentally safe and does not
degrade or release chemicals. Mr. Robinson requested Ms. Gay to inspect one of the
outfalls needing repairs and provide an estimate of the typical price to repair one
structure. Mr. Triche then reviewed Harris County Flood Control specifications and
pricing for maintaining and replacing outfall structures. Mr. Robinson recommended
Ms. Gay and Mr. Triche provide information to Mr. Mueller and Mr. Mueller prepare a
study to help the Board in determining a plan regarding replacement or repair.
BOOKKEEPER'S REPORT
The Board reviewed the tax assessor's report, a copy of which is attached. The
Board also reviewed the delinquent tax collections report, a copy of which is attached.
Director Robinson moved to approve the report. Director Renberg seconded the
motion, which carried unanimously.
Mr. Tiff reviewed the operator's report with the Board, a copy of which is
attached.
After review and discussiory Director Renberg moved to approve the operator's
report, the May mowing, sign placement, sinkhole repairs, and the action items.
Director Kainer seconded the motiory which carried unanimously.

ENGINEER'S REPORT
Mr. Mueller submitted to the Board the written engineer's report, a copy of
which is attached.
Mr. Mueller updated the Board on the Grand Lakes development and stated he
had no items for the Board's approval.
Mr. Mueller updated the Board on Meadowbrook Farms and stated he had no
items for the Board's approval.
Mr. Mueller stated that the U.S. Army Corps of Engineers ("Corps") has
requested a typical cross section of all the ditches to be covered by the jurisdictional
permit. He stated the Corps review will take approximately 9 to 12 months. He stated
the District will be required to post a notice to all residents in the District when the
application is posted on the Corps website to request comments'
Mr. Mueller stated AECOM received a request from the property owner
association for the District to construct a sidewalk along the south side of Westheimer
Parkway crossing Ditch VA1. He stated the estimated cost is between $3,400 and
$4,500. Mr. Robinson stated since the District does not collect water and sewer revenue/
he will have to research whether the District can construct the sidewalk.
Mr. Mueller stated no additional silt removal should be done until the new
permit is received.
Mr. Mueller next reviewed the updated MS4 year two action items. Director
Renberg stated she knows a group interested in installing the inlet and manhole
markers. Mr. Mueller stated the estimate to translate the construction brochure is
$3,000. The Board requested Ms. Hopper to soiicit a proposal from another translator.
Mr. Mueller then reviewed an example of a protocol to detect and eliminate elicit
discharges. He also reviewed a sample checklist for reviewing plans and a construction
site inspection form.
Mr. Mueller updated the Board on the status of the new drainage model.
After review and discussion, Director Kainer moved to approve the engineer's
report and action items. Director Lyons seconded the motion, which passed by
unanimous vote.
ACTION ITEMS
The Board then reviewed and discussed the action items list.

The Board of Directors of Willow Fork Drainage District will hold a regular meeting on
Wednesday, April 29, 2009 at 11:30 a.m., at the Willow Fork Country Club, 2'1.055 Westheimer
Parkway, Katy, Texas, to discuss and, if appropriate, act upon the following items:
1. Approve minutes.
2. Comments from District residents, including requests for consents to
encroachment or temporary use of District facilities.
3. Security services, fines and penalties for damages to District facilities and violation
of District regulations.
4. Maintenance of storm sewer lines, including repair of sinkholes and televising of
outfalls and repair options for outfalls.
5. Financial and bookkeeping matters, including payment of the bills and review of
investments.
6. Tax assessment and collections matters, including delinquent tax collections,
installment agreements, and payment of tax bills.
7. Operation of District facilities, repairs and maintenance, including ditch
maintenance and desilting projects.
8. Engineering matters, including:

We have the requested reservation of drainage capacity below for the Board's approval.
8f of District Facilities
1.
For the Jurisdictional Waters Permit, the U.S. Army Corps of Engineers (USACE) requested a typical cross section of
all the ditches, this was received by the USACE on March 18, 2009. The USACE will require the District to post a
notice to all residents in the District when the application is posted on their wesite to request any public commments.
We are coordinating with the USACE and the operator on what will be required to be sent out and that it can be
included on the water bills.
2. We have received a request for the District to construct a S-foot wide concrete sidewalk along the south side of
Westheimer Parkway, crossing Ditch VAI . We have received an estimate for this cost in the amount of $4.SO/square
foot, which would increase to $6.00/square foot is sodding is needed. The cost is estimated to be between $3,400 and
$4,500. Cinco MUD No. 'l will also be constructing a sidewalk in the same area if the Board would like to combine
these two projects.
MS4 Matters

tube of adhesive. ln addition, we included a draft introduction letter, draft instruction page, a draft group registration
form, and a draft safety procedures that would be included in the kit for your review. We have received all of the
markers the District ordered.
2. We are presenting to the Board the Construction Brochure (BMP 4.3.4) and a Post Construction Brochure (BMP 5.3.3).
We have attached a copy. We did look into a Spanish version of the Construction Brochure and have received a verbal
quote of approximately $3,000 to translate the construction and post construction brochure into Spanish by SL
lnternational. These two brochures would be distributed to the contractor so they can help maintain the project site
during and after construction.
3. For Permit Year 2, we need to develop a protocol regarding an illicit discharge detection and elimination (BMP 3.3.3).
We have attached a copy explaining how to go about in investigating illicit discharges and how to do the sampling. We
have received comments from the Attorney and the Operator and finalized the protocol for the Board's approval.
4. For Permit Year 2, we need to develop a plan set review checklist that will aid us in reviewing a set of plans (BMP
4.3.1 , 4.3.2,5.3.2). The checklist is attached for the Board's approval.
5. We have attached a Construction Site Inspection Form for the Board's review and approval. This document, BMP
4.3.2, is be used for Construction Site Inspections where the Operator will be inspecting construction site within Willow
Fork Drainage District.
6. We have attached an updated checklist for Permit Year 2 items. We will need the Board's approval to release the
checklist to the other Cinco MUDs.

First, let me extend a heartfelt thank you for your group's assistance in installing storm water inlet
decals for \Mllow Fork Drainage District. Our Storm Water Inlet Decal Program is one aspect of our
TPDES Phase ll Storm Water Permit (TXR040196). Our Phase ll Storm Water Permit requires us to
create and implement a Storm Water Management Program (SWMP) which details the steps that we
will take to reduce or eliminate pollutants in storm water runoff.
With the implementation of the Storm Water Inlet Decal Program, Willow Fork Drainage District comes
closer to completing one of the Public Education and Outreach goals stated in the SWMP. The Public
Education and Outreach portion of the SWMP entails the distribution of educational materials to the
community or conducting equivalent outreach activities. Below is a brief explanation of the purpose of
the Storm Water Inlet Decal Program.
This Storm Water Inlet Decal kit provides all the materials necessary to assist with the program.
Please review with your group the documents provided within the kit such as the instructions on
installation, safety procedures, and tips. A good estimate for applying markers is 1 marker every 3
minutes, if working in groups ol2 or 3. For a kit of 30 decals, please allocate an hour and a half for
application. The following items are provided in the kit:

The das Curb Decal should be applied to flat, non-soil surfaces and applied when the
temperature ranges from 40-90" F.
1. Glean Surface
Make sure the application surface is flat, dry and free of any
loose debris. Use the provided brush to remove dirt and
sediment from the application surface.
2. Apply Adhesive
Using the provided adhesive, coming in 1/8" from the outside
edge of the decal, apply a bead of adhesive following the
adhesive guide towards the center.
3. Stick lt!
Push the decal down on the application surface forcing a bead
of adhesive out around the entire edge. lt is important that the
entire edge of the decal is sealed so do not remove excess
adhesive from around the decal.
As noted in the Introduction letter, please denote on the Map of Willow Fork Drainage District
Inlet Locations where decals are installed. When installation is complete, return the marked
map to AECOM at the address listed below:

$444. Demand letters have been sent, we are trying to reach each owner by phone and we are contacting
mortgage companies on the real property accounts. Unless the Board would like this handled another
way, we plan to continue collection efforts and consider filing suit in July when the 2008 taxes are
turned over to us for collection.

46r.2 / Materials. Metal pipe shall be full circle conforming to AASHTO M36,
or AASHTO M196, Typ. I as specified on the plans.
The shape and minimum gage for pipe shall conform to Height of Cover
Tables for ted the ional
NCSPA
and out with a bituminous coating which shall meet the performance
requirements set forth herein and with AASHTO M-190. The pipe shall
be uniformly coated inside and out to a minimum thickness of 0.05-
inch, measured on the crests of the corrugation.
For pipe 3O-inch and larger, the minimum width of the trench shall be
the width of the outside barrel of the pipe plus l6-inches and the
maximum width of the outside barrel of the pipe plus 24-inches.
After the trench has been excavated to the bottom. the trench shall be
fine graded to the established subgrade. Any over excavation of the
subgrade shall be filled with 1.5 sack/ton of cement stabilized sand.
The Engineer will furnish and set vertical and horizontal control stakes
which will establish the base line or centerline and bench marks and will
furnish the Contractor with all necessary information relating to lines
and grades. The Contractor shall establish the grade line in the trench
from grade stakes. The Contractor shall maintain this grade control a

minimum of 100-feet behind and ahead of the pipe laying operation.
The Contractor shall, at his own expense, furnish and place in position
as directed by the Engineer all necessary stakes, grade and batter boards
for locating the work. The Contractor may, at his own expense, use a
laser beam to maintain line and grade of the work as authorized by the
Engineer.

After the metal pipe structure has been completely assembled on the
proper line and grade and headwalls constructed when required by the
drawing details, selected material from excavation or borrow shall be
placed along both sides of the completed structure(s) equally, in uniform
layers not exceeding 6-inches in depth (loose measurement), wetted if
required and thoroughly compacted between adiacent structures and
between the structure(s) and between the sides of the trench, or for a
distance each side of the structure equal to the diameter of the pipe.
Backfill material shall be compacted to the same density requirements as
specified for the adjoining section of the embankment. Above the three-
fourths point of the structure, the fill shall be placed uniformly on each
side of the pipe in layers not to exceed l2-inches. For backfilling, until a
minimum cover of l2-inches is obtained, only hand operated tamping
equipment will be allowed within vertical planes two feet beyond the
horizontal projection of the outside surfaces of the structure.
461.5 Payment.__ Payment for corrugated metal pipe measured and prescribed
above, will be made at the contract unit price bid per linear foot for the
various sizes of "Corrugated Metal Pipe" of the material and protective
coating as indicated on the plans.
