Uniform Declaration

It is your responsibility to ensure that any uniform provided to you is

looked after and is always clean, well ironed and worn correctly.

Upon the termination of your employment you must return all uniforms
to (name).

Any items of uniform that are not returned or are damaged will involve a
deduction of the cost from my final wage.

I understand that any uniforms provided by (your business name) are

for the sole purpose of my employment and for no other reasons.
