Subject: Mistake on transferring of patient from ER to Extension one


Date & Time: July 3, 2010 between 9:00 PM to 9:30 PM

Persons involved: Leonel S. Miranda RN (ER Nurse)


Glen Barraquio (orderly)

The orderly (Glen) came into the ER with a wheelchair and upon seeing him I pointed him the location of
the patient “yung nasa gitnang bed, sa room 212”, I said and he went on. As I was busy referring the
patient to the Attending Physician through our ER-cellphone, I did not notice that the orderly was gone
already (curtains of the second and third bed of the ER were down during that time). I went up and
dropped by the nursing office to inform the nurse supervisor and also to use the globe cellphone to inform
the attending physician regarding the patient. While doing so, a nurse from Extension 1 came to the office
and approached me about my patient. I walked towards the Extension 1 ward and confirmed that it was
not the patient who was supposed to be on that ward. I went down and notified Mr. Glen Barraquio that
he has transferred the wrong patient. He defensively said “Sabi mo yung nasa dulong bed, kaya pala
walang swero”,and accused us of chattering around and I replied “wala akong sinabing dulo, ang sabi ko
yung nasa gitnang bed” and then I approached the other orderly and told him to get the right patient in the
patient and tranfer it to the ward and then bring back the other patient down to the ER. I talked to the
patients relative and she told me that prior to transferring she asked the orderly why was her patient
transferred to the room so soon knowing that the patient does not have an IV line yet? The orderly replied
back to and said “dun na sa kwarto yan ilalagay, sila na bahala”. I explained to the relative the incident
and apologized for the misunderstanding.

There has been a confrontation between me, Glen Barraquio & Rain De Leon (ER staff nurse) in the ER.
It was another incident that occurred after our argument. This will be filed by my co –staff.

Incident Report

On the night of July 3, 2010 around 9:15 PM, we staff nurses of the Emergency Room received a complaint
from the relative of Luningning Almira, a patient in SRCH currently admitted in Room 321 for the reason that the
patient was taken to the wrong room (212) by the said personnel. The patient does not have an IV line, and was not
yet accompanied by the ER-Nurse on duty (Leonel Miranda RN).

The patients relative (Regina Chavez) asked us regarding proper coordination between hospital staff as well
as the preparation of patients before admission & transfer to their designated room. Due to this incident, I prompted
to call the orderly personnel namely Glenn Barraquio (via 101-admitting section) to clarify the incident that has
happened.

The phone was answered by the personnel in the admitting section & I asked him if I can talk to the said
personnel, when Mr. Barraquio answered the phone I asked him if he asked the patient if they were the ones to be
taken to 212,he said “ Sa inyo ako nagtanong”. I followed up my question by asking “Sa inyo?, sino sa amin?”. He
answered “Kay ano… kay “Tukmol” . I followed up the question by asking him “Sinong “tukmol”? walang
“Tukmol” dito”. He answered “Dun sa chinito, kay Ronnel”, I asked him again “Sinong Ronnel?” that was when he
said a very strong offensive statement against me, he allegedly said “ Tang-inang to Gago” then he hung up the
phone. After a few minutes, he came rushing inside the ER, in a fulminating manner & asked “ Ikaw ba yung nasa
phone?” I answered “oo, ako yun”. Then he confronted Mr. Leonel Miranda. Due to the uncompelling emotions &
humiliation that I felt after he said those vile words to me, I told him “Ikaw Kuya, wag mo akong mumurahin at
hindi mo ako pinapalamon”. After that, he leaned forward to me in a very offensive manner & asked “Bakit ha?!”
He was then pulled away by the Respiratory Therapist on duty outside of the room. If the RT didn’t pull him away,
he undoubtly would have physically assaulted me.

This was not the first time that this kind of incident has happened to me. Last February 2010, Mr.
Barraquio also committed the same mistake by transferring a wrong patient from the Surgery ward to the Operating
Room. During that time, he did not clarify to me( I was the Ward nurse on duty that time) who was the patient to be
taken to the OR. Because “pakikisama” to my co-worker, I did not react to what has happened even though I was
the one reprimanded by my Head Nurse(Geraldine Reyes RN) because of that incident.
