Definition
Confusion is the state of uncertainty about what is happening, intended, or what is required,
resulting in loss of orientation, ability to place oneself correctly by time, location and personal
identity. (Psora)

Causes
Confusion has multiple causes, including-

Environmental factors
 Altitude sickness (Psora)
 Dehydration (Psora/Causa occasionalis)
 Heatstroke (Psora)
 Hypothermia (Psora)

Injuries
 Head injury (Causa occasionalis)

Medical conditions
 Arrhythmia (Psora)
 Brain tumor (Psora/ Sycosis/ Syphilis)
 Complications from diabetes (Psora/ Syphilis)
 Decreased hearing or vision (Psora/ Syphilis)
 Decreased or blocked blood flow to the brain, as during a transient ischemic attack
(TIA) or a stroke (Psora/ Syphilis)
 Electrolyte imbalances, such as low levels of sodium and potassium in the blood

 Low oxygen levels from chronic obstructive pulmonary disease (COPD), pulmonary
embolism (PE) (Psora/ Syphilis)
 Malnutrition and vitamin deficiencies (Psora)
 Mental health problems, such as depression or schizophrenia (Psora/ Syphilis)
 Metabolism problems from liver or kidney failure (Psora/ Syphilis)
 Seizure disorder (Psora)

 Sleep problems, such as the inability to fall asleep, insomnia or sleep apnea (Psora)

Pathophysiology
Confusion may arise suddenly or develop gradually over time. Confusion is the result of severe
brain dysfunction (Psora). The following factors are implicated in cerebral perfusion
abnormalities associated with brain dysfunction-

 Activation of microglia and neuronal apoptosis (Psora/ Syphilis)


 Brain atrophy (Syphilis)
 Decreased availability of acetylcholine (Psora)
 Dopamine, norepinephrine, and serotonin excess and their respective amino acid
precursors (Psora/ Sycosis)
 Endothelial dysfunction (Psora)
 Increased blood-brain barrier permeability (Psora)
 Reduced blood flow (Psora/ Syphilis)
 Synaptic and neurochemical disturbances (Psora)
 The response of the brain to inflammation (Psora)
 White matter lesions (Syphilis)

Endothelial dysfunction, increased blood-brain barrier permeability, and reduced blood flow
have been implicated in cerebral perfusion abnormalities associated with brain dysfunction.
The response of the brain to inflammation, including activation of microglia and neuronal
apoptosis, leads to synaptic and neurochemical disturbances. Decreased availability of
acetylcholine during critical illness leads to decreased counter-regulatory activity in response
to inflammatory disease states, causing additional injury and further neurotransmitter
imbalances. Dopamine, norepinephrine, and serotonin excess and their respective amino acid
precursors are also associated with brain dysfunction.

Sign and symptoms


Confusion is a change in mental status in which a person is not able to think with his or her
usual level of clarity. Frequently, confusion leads to-

 Alteration in the sleep - wake cycle (Psora)


 Altered cognition, general behavior, motor activity and features of psychotic or
neurotic conditions (Psora)
 Confusion for time, place, person (Psora)
 Distress (Psora)
 Feelings of disorientation (Psora)
 Impaired decision-making ability (Psora/ Syphilis)
 Inability tell correct time and the date (Psora)
 Memory disturbance (Psora/ Syphilis)
 Severity of confusion may fluctuate during the course of the day (Psora)
 The loss of ability to recognize people and or places (Psora)
 Thinking becomes disorganized (Psora)

Type of confusion
Confusion may be primary or secondary-

Primary confusion
Typically, the onset of primary conditions is insidious. Primary confusion is caused by a direct
pathology in the central nervous system-

 Alzheimer's disease (Syphilis)

 Creutzfeldt-Jakob Disease (Psora/ Syphilis)

Secondary confusion
Secondary confusion is caused by something else-

On the basis of severity, confusion may be of the following types-

Mild Confusion
In mild confusion there may be disorientation for time, this means the patient does not know
where they are in time. This may be for the hour, day, month or year. (Psora)

Acute Confusion
Acute confusion or delirium has been defined as a state of altered consciousness in which
patients are bewildered and misinterpret the world around them. (Psora)

Severe Confusion
In more severe states there is disorientation for person, for example nurses may be believed
to be members of the patient’s family. (Psora/ Syphilis)

On the basis of stage of confusion, it may be of the following types-

Progressive confusion
The severity of confusion may fluctuate during the course of the day. As the condition becomes
more severe place is also confused so the individual does not know where they are, often
believing they are somewhere else. (Psora)

Delirium
Delirium is characterized by a disturbance of consciousness and a change in cognition that
develop over a short period of time. The disorder has a tendency to fluctuate during the course
of the day, and there is evidence from the history, examination or investigations that the
delirium is direct consequence of a general medical condition, drug withdrawal or
intoxication. (Psora)

Delirium tremens
Delirium tremens (DTs) is the most severe form of ethanol withdrawal manifested by altered
mental status (global confusion) and sympathetic overdrive (autonomic hyperactivity), which
can progress to cardiovascular collapse. (Psora/ Causa occasionalis)

Decreased level of consciousness


When consciousness is decreased, ability to remain awake, aware, and oriented is impaired.
(Psora/ Syphilis)

Unresponsiveness
It is the quality of being unresponsive, not reacting, as a quality of people, it is marked by a
failure to respond quickly or with emotion to people or events. (Psora/ Syphilis)

Intermittent confusion
Intermittent confusion is a mental state that comes and goes in which a person is disorientated
to time, person, or place. (Psora)

Diagnosis
General examination
 Vital signs including orthostatic vital signs
 Complete neurologic exam including- deep tendon reflexes (DTR’s), Cranial nerves
exam (CN2-12), motor, sensory, gait
 Carotid and cardiac exam
 Lung exam
 Exam of the lower extremities

Diagnosis is made by use of a mental test score tool. Diagnostic criteria from the American
Psychiatric Association for delirium is as below-

 Disturbance of consciousness (i.e. reduced clarity of awareness of the environment)


with reduced ability to focus, sustain or shift attention.
 A change in cognition (such as memory defect, disorientation, language disturbance)
or the development of a perceptual disturbance that is not better accounted for by a
pre-existing or evolving dementia.
 The disturbance develops over a short period of time (usually hours to days) and tends
to fluctuate during the course of the day.
 There is evidence from the history, physical examination, or laboratory findings that
the disturbance is caused by the direct physiological consequences of a general
medical condition, substance intoxication or substance withdrawal.
