Have all the identified possible causes been thoroughly investigated ?
Is the topic relevant with the organization objectives? Do all the actions link to the root cause of the problem ?
Is there any other reason to work on this topic (e.g., learning purposes, …) ? Who is responsible for doing what by when ?
Will these actions items prevent recurrence of the problem ?
Is the implementation order clear and reasonable ?

What is the current State ?(Grasp the Situation ,Use Pareto, etc.)

Is the current condition clear and logically depicted in a visual manner ?


How could the current condition be made more clear for the audience ?
Does the current condition depiction frame a problem/situation to be resolved ?
What is the actual problem in the current condition ?
Are the facts of the situation clearly established, or are there opinions ?
Is the problem quantified in some manner or is it too qualitative ? (Can you turn on and off the problem)

Countermeasures - Action plan

Are there clear countermeasures steps identified ?


Do the countermeasures link to the root cause of the problem ?
Are the countermeasures focused on the right areas ?
Who is responsible for doing what by when ?
Will these actions items prevent recurrence of the problem ?
What is the target performance ? Is the implementation order clear and reasonable ?
Is there a clear goal or target ? How will you measure the effectiveness of the countermeasures ?
What, specifically, to be accomplished ? Does the check item aligned with the previous target statement ?
How will this goal be measured or evaluated ? Has actual performance moved in line with the target statement ?
What will improve, by how much and when ? If performance has not been improvedn then why ? What was missed ? ( revisit the root cause Analysis section)

Which temporary measures have been taken ?

Root Cause Analysis (use tools, i.e., Fishbone, 5 Whys, …)

Is the analysis comprehensive at a broad level ?


Is the analysis detailed enough and did it probe deeply enough on the right issue ?
Is there evidence of proper 5-whys thinking about the true cause ?
Are all the relevant factors considered (Fishbone- Manpower, machine, material, method, environment) ?
Has cause and effect been demonstrated or linked in some manner ? Follow-up
Have all the identified possible causes been thoroughly investigated ?
What is necessary to prevent recurrence of the problem ?
What remains to be accomplished ?
What other parts of the organization need be informed of this result ?
How will this be standardized and communicated ?
