Summit Fitness Center - Membership FAQs

As of January 3, 2018

1. What offerings and programs are available in the Summit Fitness Center?
The Summit Fitness Center offers a variety of classes and resources to keep you active! So many
options are available with your membership, including:

 12 FREE group exercise classes per week


 Variety of specialty series and Wellness Programs
 3 Trainers on-site for Personal Training and Consultations
 Massage Therapist on-site by appointment only
 On-site 5Ks & Open Houses

Some additional costs may apply.

2. When does 2018 Fitness Open Enrollment begin?


Open Enrollment for 2018 begins Wednesday, January 3 and ends Friday, February 23, 2018.

3. What if I miss the Fitness Open Enrollment time frame, can I still join?
Yes, however you will not be eligible under the subsidy unless you never have had a membership. If
you are attempting to re-activate your previous subsidized membership, you will only be able to do
so January 3 – February 23, 2018.

4. If I was a member previously but lost the subsidy, do I need to completely re-enroll?
No, you should contact Summit Fitness Center by email at Kiley.Mutschler@activewellness.com,
and request to REACTIVATE your previous membership. You will be notified via email upon
reactivation, please allow 24 hours for completion. For our own tracking records, we cannot accept
verbal requests, please make your request via email.

NOTE: Scanning your AMD photo badge at the Fitness Center entrance card reader will NOT count
towards your active membership usage.

5. If I am a current member and have never lost my subsidy eligibility, do I need to take any action
to remain enrolled as a member?
No action is required. Your current account will remain open and valid.

6. I lost my subsidy in previous years and began to pay directly (Self-Pay Membership), can I convert
my membership back to an AMD-subsidized membership?
Yes, employees and family members that previously converted to Self-Pay Membership due to
inactivity have been automatically converted back to Subsidized Membership as of January 1, 2018.

any questions about your membership change.

7. If I have NEVER been a member and/or subsidized member, how do I become one?
Please complete online enrollment at Summit Health & Fitness Center Online Enrollment. If you do
not have one on file, please also complete an AMD Waiver of Liability and email it to
Terry.Hile@amd.com for processing.
Please allow 24–28 hours for the enrollment to be processed. You will be notified via email that
your Active Wellness membership badge is available for pick up. Your AMD photo badge will also be
granted fitness center entry door access following the enrollment process.

8. Will I need to request a new Active Wellness membership badge if I am reactivated as a


subsidized member?
No, after reactivation, your previous membership badge will remain valid. However, please request
a new RED Active Wellness membership badge if you no longer have your original one.

9. Is there a cost for a replacement membership badge?


No

10. Who is eligible to be a subsidized member of the Summit Fitness Center?


AMD Employees and Employee Family members are eligible. Family members are defined as
spouse, domestic partner, child(ren) over the age of 18, and live-at-home parent of the Employee.

Contractors are eligible to receive and maintain Self-Pay membership, but their membership fees
will not be subsidized by AMD. Contractors have the option to self-pay $25/month directly to Active
Wellness.

11. As a paying Contractor, are my family members also eligible to join if they pay the monthly
membership fee?
No, the Fitness Center is designed to accommodate all tenants on campus and with the increasing
number of tenants/companies on-site, HPI/Spear Street cannot extend memberships to contractor
family members.

12. As a subsidized member, how do I maintain an active membership paid for by AMD?
Using the Summit Fitness Center four (4) times (or more) per calendar quarter will allow eligible
members to maintain their AMD-subsidized membership.

13. How does my fitness center usage get tracked?


Everyone who joins the fitness center receives a RED Active Wellness membership card to the
Summit Fitness Center, different than the AMD photo badge. This card needs to be scanned into
the Member Software computer located at the fitness center entrance upon arrival only.

NOTE: Scanning your AMD photo badge at the Fitness Center entrance card reader will NOT count
towards your active membership usage.

14. What if I am unable to maintain the four (4) times per quarter criteria?
Reports are provided to AMD at the end of each quarter. Any member that did not use the fitness
center 4 times per quarter will be notified via email and removed from the eligible member list. At
that time, AMD will not pay for future months of usage during the current calendar year, but the
individual is able to self-pay for membership.

15. Once I have been removed from the subsidy for inactivity, can I continue to use the fitness
center?
Yes, you can continue to use the fitness center; however, your membership will not be subsidized
by AMD for the remainder of the calendar year. You will be required to pay the $25/month
membership fee directly to Active Wellness to continue usage in the current year.
16. Are there Qualifying Events that allow me to place my subsidized membership on hold if I know I
will not be able to meet 4 times/quarter criteria?
Yes, there are situations that affect a member from being able to meet the eligibility criteria.
Qualifying events that allow AMD to hold/freeze your membership are:
 AMD Approved Leave of Absence (i.e., family or medical/maternity leave, military, and
personal)
 Sabbatical
 AMD Business travel more than 4–6 weeks within the quarter

Please contact Kristol.Wayman@amd.com in AMD Global Real Estate & Workplace Services if you
need to report one of these qualifying events. Prior notification is preferred when possible.

17. Can I opt-out of the subsidized program and pay fees directly when I want? If so, how much is the
monthly membership fee?
Yes, eligible members can choose not to participate in the AMD subsidized program and can enroll
as paying members directly with Active Wellness for $25/month. Cancelling or Opting-out of the
subsidy is final for the remainder of the calendar year, you will not be eligible to reactivate the
subsidy at a later date within same year.

18. As a Self-Pay member, am I held to any usage parameters?


No, there are no usage parameters to meet when paying for your own membership, although you
are required to continue to meet other eligibility criteria, such as remaining employed with AMD.

19. How do I set up payment to Active Wellness? When are monthly fees collected?
Monthly fees are due at the first of the month for the current month. A personal credit card must
be provided and Active Wellness will establish auto-payment.

20. For Contractors and Self-Pay members: Are there pro-rated rates for joining late in the month?
Yes, members who join after the 25th of the month will not be charged until the following month.
Members who choose to cancel their membership on their own, will not be charged for that month
of membership if the cancellation is completed by the 15th of the month. Cancellations after the
15th of the month will be charged for the then-current month, with the cancellation effective in the
next month.

21. If I have a question about how much I’ve utilized the fitness center, who can I ask?
Members can track their own usage by using Mind Body Online:

 Sections labeled "New to our site?" enter "First Name" & "Last Name"
 Select button labeled "Next"
 Please follow steps on each succeeding screen
 On screen labeled "Personal" select link labeled "Edit"
 Please update all information & answer all listed questions (Yes or No)
 Select button labeled "Save"

As always, Active Wellness staff at the Summit Fitness Center can provide assistance or answer any
questions.
22. Will I get notices if I haven’t been using the fitness center enough to remain active?
Yes, reports will be generated at the beginning of each month and members will be notified via
email of “low usage” so that they are aware of their status on a monthly basis.

23. Can I travel from another site and use the Summit Fitness Center for a short period of time as a
Guest?
Yes, AMD Employees traveling to Austin from another site may complete a guest waiver and get a
temporary badge to the Fitness Center that must be returned upon departure. Guest badges are
issued by HPI and are valid for 5 days. There is no charge to be a guest under this program.

24. Will Open Enrollment be available every January?


The program will be evaluated during each calendar year to determine how to proceed in the
future. AMDers will be notified of any program changes once confirmed. If any action is required,
ample notice will be provided.
