years old, married, presently assigned at La union PS and a resident of barangay Minante 1,
Cauayan City, Isabela whether the person is firearm license holder or of any kind of firearms in
your office after confiscated from his direct possession and control one (1) 9mm caliber pistol
beretta with serial number ABC 12345 and one (1) magazine loaded with 12 lives Ammos. and
likewise to determine the identity of the registered owner.

3. Further request furnish this office a copy of the result of your verification.

licensed/registered firearm holder of caliber 9mm beretta pistol with serial number ABC 12345
as of this date.

3. Further be informed that as of this date, caliber 9mm beretta pistol with serial
number ABC 12345 is a licensed/registered firearm per verification from records of this office.
