plate no. HG-7291 and WAVE bearing a temporary plate no.

9008-0056015.
III. FACTS OF THE CASE:

7. On or about 2:30 in the afternoon of September 16, 2017 a bystander

who saw the incident and reported and ask police assistance on the alleged motor
accident at Emilda park Catbalogan City. PO3 Lim-im it Roger the first responder
who arrived and secure the crime scene.

Siago Jezoriel 19 years old single, jobless a resident of Brgy. Canlapwas

Catbalogan City (witness) he saw XRM bearing a plate no. HG-7291 driving fast by
Aguilar Angelito coming from the right and unfortunately a WAVE bearing a
temporary plate no. 9008-0056015 driving by Bacarra Alfred U-turn to the direction
of the first motor vehicle and accidentally collide each other and the two driver lay
down in the side of the street, Dacuma Arvin has a knowledge about first aid he try to
save the life of the two drivers, unfortunately they died because of the impact of the
collision.

8. In the investigation conducted by the duty traffic investigator in the alleged

motor accident after a verification and inspection conducted XRM bearing a plate no.
HG-7291 the driver was drunk.

9. When the traffic investigator conducted a follow –up investigation in the said
vicinity the place has lack of traffic signs and slippery and narrow road and the
condition of the weather was rainy.

10. In the statement of Siago Jezoriel 19 years old single, jobless a resident of
Brgy. Canlapwas Catbalogan City on or about 2:30 PM. September 16, 2017 while
waiting his father he saw XRM bearing a plate no. HG-7291 driving fast and the other
motor vehicle U-turn and collide each other, and immediately ask assistant at the
nearest police station.

IV. DISCUSSION/FINDINGS:

11. Under the investigation conducted by the traffic investigator the driver of
XRM HG- 7291 the driver was drunk and he drive fast because of the findings of the
impact and position of the motor vehicle.

In the follow up investigation the street has lack of traffic signs narrow, and
slippery road and the condition of the weather was rainy.

V. CONCLUSION:

After a careful investigation of the scene and the testimony of the witness and
evidence gathered that the vehicle driving by Mr. Aguilar Angelito is unregistered
and the driver was drunk when the incident happened.
VI. RECOMMENDATION
 13. In the view of the foregoing, the undersigned respectfully recommends that
debris recovered and both motor vehicle will undergo field examination.

BRIEF NARRATION OF FACTS:

On or about 2:30 in the afternoon of September 16, 2017 a bystander who

saw the incident and reported and ask police assistance on the alleged motor

who arrived and secure the crime scene.

ACTION TAKEN

SPOI Diaz Eldrin and Mendes Arvin conducted investigation procedure at the
scene of incident after the victim/ driver rescued by the medication the two motor
vehicle involved was delivered to the laboratory for field examination and measured
the crime scene 83cm from the distance of the victim to the victim and 110 cm from
one vehicle to another vehicle 12 cm of the distance from the vehicle to the victim
measure by PO2 Jerry Oleo and PO3 Mendes Arvin and the rough sketch made by
PO1 Diaz Eldren.

REMARKS/DISPOSITION:

The driver was drunk when the incident happened and the street was lack of
traffic sign and narrow, slippery road.

CONCLUSION / RECOMMMENDATION

In the view of the foregoing, the undersigned respectfully recommends that

debris recovered and both motor vehicle will undergo field examination and the
driver was drunk when the incident happened.

III. FACTS OF THE CASE:

7. On or about 3:00 in the afternoon of September 20, 2017 a brgy. Tanod

named Dacuma Arvin while in the tour of his duty saw the incident happened and
immediately reported the incident and ask police assistance on the alleged motor
accident at Emilda park Catbalogan City. PO3 Lim-im it Roger the first responder
who arrived and secure the crime scene and conduct preliminary investigation.

Catbalogan City (witness) he saw WAVE bearing a plate no.19008-0056015 driving
fast by Bacarra Alfred coming from the the real street and unfortunately Aguilar
Angelito pass across in the street where he accidentally hit by Bacarra Alfred at
emilda park and the driver f the said vehicle was died on arrival.

8. In the investigation conducted by the duty traffic investigator in the alleged

motor accident after a verification and inspection conducted WAVE bearing a plate
no.9008-0056015 the driver was drunk and no handbrake.

9. When the traffic investigator conducted a follow –up investigation in the said
vicinity the place has lack of traffic signs and slippery and narrow road and the
condition of the weather was rainy.

10. In the statement of Siago Jezoriel 19 years old single, jobless a resident of
Brgy.10 Catbalogan City on or about 3:00 PM. September 20, 2017 while walking at
the side walk he saw a WAVE bearing a plate no.9008-0056015 driving fast and hit a
pedestrian passing across the street and immediately ask assistant at the nearest
police station.

IV. DISCUSSION/FINDINGS:

11. Under the investigation conducted by the traffic investigator the driver of
WAVE bearing a plate no. 9008-0056015 the driver was drunk and he drive fast
because of the findings of the impact and position of the motor vehicle and damage
caused to the victim.

In the follow up investigation the street has lack of traffic signs narrow, and
slippery road and the condition of the weather was rainy.

V. CONCLUSION:

After a careful investigation of the scene and the testimony of the witness and
evidence gathered that the vehicle driving by Mr. Aguilar Angelito is unregistered
and the driver was drunk when the incident happened.
VI. RECOMMENDATION

debris recovered and motor vehicle will undergo field examination.

ACTION TAKEN

SPOI Diaz Eldrin and Mendes Arvin conducted investigation procedure at the
scene of incident after the victim/ driver rescued by the medication unfortunately
died on arrival the motor vehicle involved was delivered to the laboratory for field
examination and make rough sketch and measured the distance, 100 cm from the
victim to the vehicle, 65cm the distance from the victim sleeper and 65 cm to the
sidewalk.

REMARKS/DISPOSITION:

The driver was drunk when the incident happened and the street was lack of
traffic sign and narrow, slippery road.

CONCLUSION / RECOMMMENDATION

In the view of the foregoing, the undersigned respectfully recommends that

debris recovered and motor vehicle will undergo field examination and the driver
was drunk when the incident happened.
