NATURE OF FINDINGS

FOUND OUTSIDE SHELL COURSE 5 & 7 WHICH AT RING AREA WAS THINNING BELOW THE
REQUIRED 32MM THICKNESS.

IMMEDIATE REMEDIAL ACTION

MMHE-ATB HAD PERFORMED JOINT INSPECTION & REVIEW OF AFFECTED THINNING AREA
FOR CONFIRMATION. REFER BELOW THICKNESS TABULATION ON THE OUTSIDE RING AREA.

DISPOSITION ACTION

1. MMHE-ATB PROPOSED TO WELD BUTTERING AT AFFECTED THINNING AREA. THE EXTENT


AREA OF BUTTERING SHALL PROVIDE AT LEAST 100MM.

2. ORIGINAL APPROVED PROJECT ‘SAW’ WELDING PROCESS, AS PER WPS NO. MMHE-ATB-
332/346 SHALL BE USED FOR ALL BUTTERING WELDING.
3. EACH PASS OF WELDING SHALL ENSURE THOROUGH CLEANING BY POWER-TOOL
/GRINDING BETWEEN PASSES.
4. PREHEAT SHALL BE PERFORMED AS PER APPROVED WPS (MINIMUM PRE-HEAT

TEMPERATURE: 100 C).
5. THE WELD PASS SHALL BE OVERLAP AT LEAST 50% BETWEEN EACH PASSES.
6. MINIMUM 2 LAYERS OF WELD BUILD-UP SHALL BE PERFORMED IN ORDER TO PROVIDE
SUFFICEINT THICKNESS.
7. IF ANY AREA CANNOT BE COVER BY ‘SAW’ BUTTERING OR ANY TOUCH-UP REPAIR, ‘SMAW’
WELDING PROCESS SHALL BE USED AS PER WPS NO. MMHE-ATB-334/346A
8. THE BUILT-UP AREA SHALL HAVE SMOOTH PROFILE. GRINDING SHALL BE PERFORMED IF
THE PROFILE NOT SMOOTH.
9. THE THICKNESS AFTER WELD BUTTERING SHALL BE ABOVE MINIMUM REQUIREMENT
REQUIRED OF THE SHELL WHICH 32MM.
10. VISUAL INSPECTION SHALL BE CARRIED OUT TO ENSURE FREE OF SURFACE DEFECTS.
11. PERFORM 100% MPI, UT THICKNESS & UT FLAW SCAN 45° PROBE (BOTH AT AMBIENT
TEMPERATURE).
12. PERFORM TRANSVERSE HARDNESS CHECK ON THE SURFACE OF BASE METAL, HAZ, AND
WELD BUTTERING CHECK EVERY 45°.
13. ALL THE REPORTS SHALL BE ENDORSED BY MMHE-ATB/CPP/PRPC AND KEEP A RECORD:
i. VISUAL REPORT
ii. MPI REPORT
iii. UT THICKNESS GAUGING REPORT
iv. UT LAMINATION & UT FLAW USING PROBE 45°
v. HARDNESS REPORT
vi. SHELL FIBER ELONGATION
IS THIS REQUIRED ?
