Summary: Tracheostomy is a procedure for patients who are at risk of having severe hypoxia
that is related to difficulty of breathing. Many relatives said that they are satisfied of the care
given to the patient when it comes to tracheostomy care which is done by effective
collaboration of health teams. But what important is that that care will still be given to that
patient even he/she is at home. And a way to improve that is to teach the relatives on how to
take care of a patient with tracheostomy by explaining the procedure step by step. And the
participation of the patient is crucial but it can be strengthen by the Self-management support.
Explain how the role of the relatives is important in taking care so that they will be eager to
learn and also to provide an effective communication and interaction between the family and
the health care team.

- The implications of the findings to the patient care is that it indicates that it is
important to have a effective collaboration of the health care team and a good health
education to the patients and their relatives. It is very important to empower people of ways
how to prevent illness and we have a big role on that as student nurses and as future nurses of
our role as educators and when talking about patient it will improve patient care not only
inside the hospital but it will continue even at their home.

- This practices will improve patient care and outcomes because if the goal of
nurses is to provide quality and effective care to the patients might as well maximize it, and
by these interventions the care that you have given to the patient at the hospital will still
continue at the house because of proving health teaching and giving support to the family to
have eagerness in learning and applying the knowledge that was given to them. In that way it
would shorten the period of illness and also prevent further complications to the patient at
home.
8. Synthesis
The articles are about a comparative analysis between patient’s perspective of self-
management support and on teaching, transitions, and multidisciplinary teams when it comes
to long term care. These articles talks about effective communication and proper ways of
teaching a patient because this things give support to the patient and to the family in order to
give care effectively and also for their full participation. Therefore these articles support each
other in a sense that they are both crucial in improving the outcomes and shortening the time
of illness experiencing by the patient.
