Setiawan :” Good Morning nurse,”

Chana :” Well Mr,I need to collect your personal data.I’m going to ask you some question.”

Setiawan : “ Sure.”

Chana :”Can you tell me what is your name?”

Setiawan :” My name Setiawan.”

Chana : “ How old are you Mr.Setiawan?”

Setiawan :” I am 37 years old.”

Chana :” Where do you live?”

Setiawan :” I live in Serayu street.”

Chana :”Are you merrid?”

Setiawan :” Yes, I am.”

Chana :”Do you have any health insurance?”

Setiawan : “ Yes, I have. My health insurance is BPJS.”

Chana : “ What is your occupation?”

Setiawan : “ I am a businessman.”

Chana :” what is your phone number?”

Chana :” Who is your next of kin?”

Setiawan :” My wife, her name is Lilis”

Chana : “ What makes you come in the hospital?”

Setiawan :” I feel not so good, my stomach is painful.”

Chana :” Ok, first I am going to check your vital sign Mr Setiawan.”

Setiawan :” Ok.”

Nurse doing ceck Setiawan’s health condition.

Chana :” Ok,Sir! I have finish to asses your personal data and assessment your health. I will
consultation with the doctor about your condition.”

Setiawan :” Thank’s you very much, Nurse”

Chana : “ You’re welcome. I excuse my self first.

DIMENSION OF SYMTOMS

In the a hospital, a patient named is Mr Chana was checked a nurse. The Name of nurse is Setiawan.Mr
Chana suffering stomach pain and treated in the hospital for two days.

Setiawan :” Good Afternoon, Mr Chana?”

Chana :” Good Afternoon,Nurse.”

Setiawan :” What’s wrong with you? Can you tell me about ur health condition right now?”

Chana :” I feel bad, nurse. My stomach still hurt. I have not appetite. “

Setiawan :” Oh , I am sorry to hear that. May I check your condition right now?”

Chana :” Oke..”

Setiawan :” I will check your blood pleasure first, Give me your arm.. Your blood pleasure
120/90.it is normal. And next, I will check ur stomach. Where the area is sick?”

Chana : “ Here,,, left abdomen.”

Setiawan check the left abdomen Mr Chana and show pain scale images.

Setiawan : “Your scale pain is number 3. You feel like impaled by a knife.”

Chana : “ Yes.”

Setiawan : “ Whatever ever vomiting?”

Chana “ No….”

Setiawan :” Are there any other pain?”

Chana : “ No. May I get an appetitie vitamin, Nurse?”

Setiwan :” Well, I will tell to a doctor for your request. If you don’t have another pain, may I
excuse my self first?”
