Post-Practicum Paperwork: Clinical Analysis

1. Finalize the acuity list by adding or subtracting pertinent information. Are there any
changes in the acuity ranking of the assignments? Give rationale.

#1-The patient with the intracranial abscess has the highest acuity due to his cognitive ability
and confusion. He was on twenty-four hour restraints due to the multiple successes of IV
dislodging. Also, the patient was an aspiration risk which meant one on one for meals.
#2- Elderly female patient that was experiencing hallucinations – The reason she is number
two is due to her hypotensive episodes, impaired mobility and was dependent on staff.
#3- Female patient with intracranial hemorrhage- she was cognitively intact but required
assistance with adls and there needs to be heighten attention on monitor LOC with this
patient due to recurrent chance of bleeding.
#4- 60 year old Male with CVA- Due to his admitted dx. Is important to assess and observe
this patient for residual effects from the stroke such as dysphagia, muscle weakness, etc.;
Also, he may have impairments due to the diagnosed stroke.
#5- 79 y/o Male with Community acquired Pneumonia- This patient had impaired mobility,
but was high function independent, he was able to perform all adls independently or with
assist of his wife.

2. What problem(s) or concerns did you encounter as the peer leader? What did you do to
solve the problem(s)?
As a peer leader the only problem was ensuring that we finished up in a timely matter to
leave the floor for lunch. This problem can cannot be solved individual by the peer
leaders instead it’s on an individual basis. As a peer leader I guided my peers to stay on
task by reminding them to get Blood sugar checks along with vitals.

3. Describe 3 principles of servant leadership and how you portrayed each of them as a peer
leader. (Minimum 3 paragraphs. Reference in APA is required.)

As a peer leader you step into a leadership role by being the resources or helping hand for
your peers. The three principles of a peer leader I displayed were demonstrating compassion,
being an active listener, and conducting respectfully and professional with my peers. As a peer
leader I urged myself to be an active listener by attend at least three of my colleague’s morning
reports so I could firsthand have an understanding of what’s on the patient’s agenda and be a
second pair of ears for my colleague. Also, I was an active listener when my peers came to me to
ask for assistance or voiced a concern.

Throughout my day as peer leader, I conducted myself professionally and respectfully

with my colleagues and staff members on the floor. I respected each and every one of my
colleague’s time, management and input. I assisted my colleagues with hygiene care, medication

preparations and bladder scanning. As a servant leader I demonstrated compassion

towards my colleagues and patients we took during our clinical day. This can be demonstrated
simply by assisting in adl care, refreshing their water, and taking the time out to talk to the
patient.

The role of a peer leader intertwines with the role of a servant leader, both individuals
hold leadership roles and are vital in the structure and goal of cohesiveness among the team. As a
student peer leader the role helps mold you into the future charge nurse and leader within the
clinical setting. This opportunity allows for you to undergo challenges that you may witness once
you become a nurse and identify adaptive ways to solve future issues.
