Oleh karena itu, maka menjaga kesehatan tubuh selama masa kehamilan menjadi hal yang mutlak di
lakukan oleh setiap wanita hamil . Namun masalah kesehatan gigi saat hamil seringkali terabaikan.
Wanita hamil lebih senang “menyibukkan diri” dengan pemeriksaan kandungan dan USG untuk
melihat perkembangan janinnya. Walaupun memang pemeriksaan kehamilan (antenatal care)
memang perlu dilakukan secara teratur dan berkala, tapi pemeriksaan gigi juga tidak boleh
disepelekan.
Pada masa kehamilan terjadi sejumlah perubahan secara fisik maupun fisiologi. Perubahan
tersebut dapat mempengaruhi sistem dalam tubuh yang akan berdampak pula terhadap fisiologi dari
bagian-bagian tubuh termasuk rongga mulut. (Pirie M, et al, 2007) Sebagian besar keluhan pada
rongga mulut wanita hamil disebabkan oleh perubahan hormon. (Gaffield M.L. et al, 2001; Jiang P. et
al, 2008) Kehamilan secara signifikan mempengaruhi terjadinya sakit pada jaringan periodontal dan
juga mempengaruhi timbulnya ginggivitis. (Pirie M, et al, 2007) Akhir-akhir ini dari penelitian
membuktikan bahwa perubahan hormon pada wanita hamil bertanggungjawab dalam terjadinya
manifestasi tersebut. (Jiang P. et al, 2008). Perubahan fisiologis selama Kehamilan
Selama kehamilan, tubuh perempuan mengalami perubahan hormonal, imunologi, dan metabolik untuk
mendukung pertumbuhan dan perkembangan janin (Kumar dan Magon, 2012). Selama periode ini, tingkat
hormon yang disekresikan (terutama progesterone dan estrogen) meningkat secara dramatis, dan ada perubahan
besar dalam respon imun (Kumar dan Magon, 2012). Perubahan kekebalan tubuh adalah kompleks, dan dapat
disebut sebagai modulasi kekebalan, seperti di satu sisi, beberapa tingkat penekanan kekebalan diperlukan
untuk menerima janin yang sedang tumbuh dengan sistem kekebalannya sendiri yang sedang berkembang,
sedangkan di sisi lain, kekebalan yang ketat harus tetap untuk melindungi ibu dan janin dari infeksi. Sementara
banyak yang menganggap kehamilan sebagai keadaan anti-inflamasi, yang lain menganggap kehamilan sebagai
proses multi-tahap termasuk tahap peradangan pada implantasi dan partus, dan tahap anti-inflamasi pada
pertengahan kehamilan, ketika janin tumbuh dengan cepat (Mor dan Cardenas, 2010).
Dalam banyak hal, perubahan metabolik yang terkait dengan kehamilan mirip dengan yang terjadi pada sindrom
metabolik, termasuk kenaikan berat badan, peningkatan kadar glukosa darah puasa, resistensi insulin,
intoleransi glukosa, peradangan tingkat rendah, dan perubahan kadar hormon metabolik ( Fuglsang, 2008;
Newbern dan Freemark, 2011; Emanuela et al., 2012; Kumar dan Magon, 2012).
Penyakit periodontal merupakan hasil dari interaksi yang kompleks antara kerentanan host dan rongga mikro
mikro oral. Berbagai faktor dan kondisi sistemik dari host dapat mempengaruhi prevalensi, perkembangan, dan
keparahan penyakit (Kinane dan Marshall, 2001). Di antara variabel-variabel ini, hormon seks kemungkinan
mempengaruhi patogenesis penyakit periodontal (Apoorva dan Suchetha, 2010; Parker et al., 1998b; Hofbauer
dan Hedfelder, 2001), karena variasi hormonal mempengaruhi fisiologi interaksi parasit host di rongga mulut
(Amar dan Chung, 1994).
Hormon adalah molekul pengaturan spesifik yang memodulasi reproduksi, pertumbuhan, dan perkembangan.
Selain itu, hormon mempertahankan homeostasis, produksi energi, pemanfaatan dan penyimpanan (Mariotti,
1994). Androgen, estrogen, dan reseptor progesteron juga terlokalisasi dalam jaringan periodontal (Gornstein et
al., 1999). Pada wanita, estrogen dan progesteron berkontribusi pada perubahan fisiologis pada fase kehidupan
tertentu. Sebagai contoh, estrogen dapat mempengaruhi cytodifferentiation dari epitel skuamosa bertingkat,
serta sintesis dan pemeliharaan kolagen berserat (Eleni et al., 2009). Progesteron memberikan efek langsung
pada periodontium dan dapat memainkan peran penting dalam kopling reabsorpsi tulang dan pembentukan
tulang (Mascarenhas et al., 2003; Gallagher et al., 1991). Secara bersama-sama, hormon memengaruhi berbagai
jaringan dan dapat mempengaruhi kesehatan seseorang.
Periodontal disease results from a complex interplay between host susceptibility and oral cavity micro
flora. Various systemic factors and conditions of the host may affect the prevalence, progression, and
severity of the disease (Kinane and Marshall, 2001). Among these variables, sex hormones likely
influence the pathogenesis of periodontal diseases (Apoorva and Suchetha, 2010; Parker et al., 1998b;
Hofbauer and Hedfelder, 2001), as hormonal variation affects the physiology of host parasite
interactions in the oral cavity (Amar and Chung, 1994).
Hormones are specific regulatory molecules that modulate reproduction, growth, and development. In
addition, hormones maintain homeostasis, energy production, utilization and storage (Mariotti, 1994).
Androgen, estrogen, and progesterone receptors are also localized in periodontal tissues (Gornstein et
al., 1999). In women, estrogen and progesterone contribute to physiological changes at specific life
phases. For example, estrogens can influence the cytodifferentiation of stratified squamous epithelium,
as well as the synthesis and maintenance of fibrous collagen (Eleni et al., 2009). Progesterone exerts
direct effects on the periodontium and may play an important role in the coupling of bone reabsorption
and bone formation (Mascarenhas et al., 2003; Gallagher et al., 1991). Taken together, hormones
influence a variety of tissues and may influence an individual’s health.
Puberty, menstrual cycle, pregnancy, and menopause are all phases that specifically influence oral and
periodontal health in women. Increased hormonal levels during puberty affect gingival tissues and the
subgingival micro flora (Steinberg et al., 2008). For example, during puberty, Prevotella intermedia and
Capnocytophaga bacteria species emerge (Yokoyama et al., 2005; Gusberti et al., 1990). Clinically,
there may be a hyperplastic reaction of the gingiva in those areas where local bacteria deposits are
present. The inflamed tissues become deep red and may be lobulated, with ballooning distortion of the
interdental papillae. Histologically, tissue appearance is consistent with inflammatory hyperplasia
(Steinberg et al., 2008). Moreover, bleeding may occur when patients masticate or brush their teeth. In
addition to puberty-induced changes, gingival tissues are more edematous during the menstrual cycle
and erythematous before its onset. Consequently, increased gingival bleeding and exudation (Machtei
et al., 2004) has been observed during the menstrual period and is sometimes associated with slight
increases in tooth mobility (Otomo-Corgel, 2007). During pregnancy, progesterone and estrogen levels
are continuously elevated and, by the end of the third trimester, peak at 100 and 6 ng/ml, respectively.
These hormone levels are 10 and 30 times greater than the levels observed during the menstrual cycle
(Grodstein et al., 1996a). It is not surprising, then, that gingivitis is the most prevalent oral
manifestation associated with pregnancy and occurs within 30–100% of all pregnant women
(Zachariasen, 1989; Ferris, 1993). In addition to generalized gingival changes, localized gingival
enlargements like “pregnancy tumor,” “epulis gravidarum” or “pregnancy granuloma,” are also
observed. The histological appearance is similar to a pyogenic granuloma, and the enlargement is found
in up to 9.6% of women (Gornstein et al., 1999). Similar types of gingival changes are also seen in
women that are taking oral contraceptives (Otomo-Corgel, 2007). Even during menopause, when
hormonal levels decline, women experience changes in the oral mucosa, which may result in burning
sensations, altered taste perception, dryness of the mouth, or menopausal gingivostomatitis (Amar and
Chung, 1994).
Several clinical and experimental studies have concluded that subclinical infections in pregnant women
are likely the most frequent cause of low births (Gibbs, 2001; Moreu et al., 2005). In the presence of
periodontal disease, lipopolysaccharide exposure, inflammatory mediators, and cytokine production in
the maternal serum increase patient risk for poor pregnancy outcomes. Periodontal infection, as a
chronic reservoir of lipopolysaccharides, may even target the placental membranes via the bloodstream
(Offenbacher et al., 1996, 1998). Therefore, periodontal disease not only influences women, it also
affects pregnancy outcomes.
In fact, surges in the female hormones estrogen and progesterone can dilate (expand) the tiny blood
vessels in your gums, increasing blood flow. This makes gums more sensitive to the bacteria (and
associated toxins) found in the sticky dental plaque that accumulates on teeth every day. Hormones
during pregnancy can exaggerate the inflammatory response to local irritants, like plaque, causing
gums to become swollen and bleed more easily during this intense (and prolonged) hormonal surge4.
Pregnant women should also be aware that studies have suggested a link between periodontal (gum)
disease and preterm delivery. Although the exact mechanism by which this happens is unclear,
evidence suggests that the bacteria in dental plaque can reach the placenta and trigger inflammatory
responses. This causes substances to be released into the bloodstream that may in turn start labor
prematurely. Preeclampsia, a form of high blood pressure specific to pregnancy, may also be
associated with periodontal disease.
Hormonal changes don’t directly cause gum disease, but they can be one factor in periodontal disease.
Changes in the hormones (particularly estrogen and progesterone) affect the gums by changing the
blood flow to the gums and affecting the way tissues respond to plaque in the mouth, causing irritation
and often bleeding during brushing and flossing. Hormonal influences on gum health tend to affect
women more often than men, since women go through more hormone changes throughout life. While
increased risk for gum disease tends to occur most often during pregnancy and menopause, many
women experience symptoms during puberty and right before a monthly menstrual cycle. Even
women who take birth control can notice more irritation and inflammation in the gums.
a. Sistem Endokrin
- Estrogen meningkat hingga 10 kali
- Progesteron meningkat hingga 30 kali

- Estrogen and Progesterone
- Estrogen and progesterone are responsible for physiological changes in women at specific phases of
their life, starting in puberty. Estrogen induces several of the pubertal developmental changes in
females, and progesterone acts synergistically with estrogen to control the menstrual cycle and to
inhibit follitropin secretion by the anterior pituitary gland. Specifically, estrogens can influence the
cytodifferentiation of stratified squamous epithelium as well as the synthesis and maintenance of
fibrous collagen. Estrogen receptors found in osteoblast-like cells provide a mechanism for the
direct action on bone. These receptors were also located in periosteal fibroblasts, scattered
fibroblasts of the lamina propria and periodontal ligament (PDL) fibroblasts, proving the direct
action of sex hormones on different periodontal tissues.
Effects of Estrogen on the Periodontal Tissues
- Decreases keratinization while increasing epithelial glycogen that results in the diminution im
the effectiveness of the epithelial barrier
- Increases cellular proliferation in blood vessels
- Stimulates PMNL phagocytosis
- Inhibits PMNL chemotaxis
- Suppress leukocyte production from the bone marrow
- Inhibits proinflammatory cytokins released by human
- marrow cells
- Reduces T-cell mediated inflammation
- Stimulates the proliferation of the gingival fibroblasts
- Stimulates the synthesis and maturation of gingival
- connective tissues
- Increases the amount of gingival inflammation with no
- increase of plaque.
Effects of Progesterone on the Periodontal Tissues
- Increases vascular dilatation, thus increases permeability
- Increases the production of prostaglandins
- Increases PMNL and prostaglandin E2 in the gingival crevicular fluid (GCF)
- Reduces glucocorticoid anti-inflammatory effect
- Inhibits collagen and noncollagen synthesis in PDL fibroblast
- Inhibits proliferation of human gingival fibroblast proliferation
- Alters rate and pattern of collagen production in gingiva resulting in reduced repair and
maintenance potential
- Increases the metabolic breakdown of folate which is necessary for tissue maintenance and
repair.
Pregnancy
Some of the most remarkable endocrine-related oral alterations occur during pregnancy due to
increased plasma hormone levels. Progesterone and estrogen reach their peak plasma levels of 100 and
6 ng/ml respectively, by the end of the third trimester, and the potential biological impact of estrogen
and progesterone take place in periodontal tissues during this period. Pregnancy gingivitis is extremely
common occurring in arange between 30 and 100% of all pregnant women. Pinard first described this
situation in 1877 characterized with erythema, edema, hyperplasia and increased bleeding. Cases range
from mild inflammation to severe hyperplasia, pain and bleeding. Increased gingival probing depths,
increased gingival inflammation, increased gingival crevicular fluid flow increased bleeding upon
probing and increased tooth mobility are the clinical periodontal manifestations that have been
described during pregnancy. The anterior region of the mouth is more commonly affected and the
interproximal sites tend to be the most involved areas. Gingival inflammatory changes in pregnancy
usually begin. during the second month and the severity of the disease increase. through the eight
month, after which there is an abrupt decrease related to a concomitant reduction in sex steroid
hormone secretion. Moreover, it has been confirmed that during pregnancy the severity of gingival
inflammation is correlated to elevations of sex steroid hormones and is reduced following parturition
and the concomitant drop-off in hormone production. There is also an increased incidence of pyogenic
granulomas during pregnancy at a prevalence of 0.2 to 9.6%. The ‘pregnancy tumor’ or ‘pregnancy-
associated pyogenic granuloma’ appears most commonly during the second or the third month of
pregnancy. Gingiva is the most common site involved (70%) followed by tongue, lips, buccal mucosa
and the palate. The pregnancy tumor develops as a result of an exaggerated inflammatory response to
local irritations, then enlarges rapidly and bleeds easily, and becomes hyperplastic and nodular. The
tumor may be sessile or pedunculated and may range from purplish red to deep blue in color with small
fibrin spots. Increased sex steroid hormones have effects on gingival vasculature, subgingival
microbiata, specific cells of periodontium and local immune system during pregnancy. Increased
edema, erythema, gingival crevicular exudate and hemorrhagic gingival tissues may also be observed
due to the effects of estrogen and progesterone on the gingival vasculature.
Kornman and Loesche reported that increased levels of estrogen and progesterone paralleled gingival
conditions and the proportions of P. intermedia during pregnancy. During the second trimester, an
increase in gingivitis and gingival bleeding without an increase in plaque levels have been reported and
further a 55-fold of an increase in the proportion of P. intermedia has been reported in pregnant women
compared to nonpregnant controls
Clinical and Microbial Changes in the Periodontal
Tissues during Pregnancy
- Increased gingival probing depths
- Increased gingival inflammation
- Increased gingival crevicular fluid flow
- Increased bleeding upon probing
- Increased tooth mobility
- Increased incidences of pyogenic granulomas
- Increased numbers of periodontopathogens especially P. gingivalis and P. intermedia
Estrogen, progesterone, and chorionic gonadotropin, all affect the microcirculatory system,
producing the following changes: Swelling of endothelial cells and pericytes of the venules,
adherence of granulocytes and platelets to the vessel walls, formation of microthrombi, disruption
of perivascular mast cells, increased vascular permeability, and vascular proliferation.[31,32,33]
Fluctuation of sex hormones during menopause have been implicated as a factor for inflammatory
changes in the human gingiva, hypertrophy or atrophy. Estrogen affects cellular proliferation,
differentiation, and keratinization of the gingival epithelium. Hormone receptors have been
identified in the basal and spinous layers of the epithelium and also in the connective tissue
implicating the gingiva and oral tissue.[34]
- Estrogen dan progesteron bersifat antagonis terhadap insulin. Sehingga untuk mengkompensasi pe
ningkatan estrogen dan progesteron tubuh meningkatkan produksi insulin. Namun sekit
45% wanita hamil gagal untuk mengkompensasi hal ini sehingga timbullah diabetes gestational.
- Esterogen dan progesteron menyebabkan pelebaran pembuluh darah sehingga sering terjadi
pembengkakan pada gusi wanita hamil, sehingga harus berhati -hati dalam melakukan prosedur gigi.

B. Pengaruh Kehamilan Terhadap Kesehatan Gigi dan Mulut
1. Terhadap Jaringan periodontal
a. Pregnancy Ginggivitis / gingivitis gravidarum
Ginggivitis adalah inflamasi pada ginggiva akibat plak pada sebagian dari mukosa mulut yang
mengelilingi gigi dan yang menutupi tulang alveolar. Selama hamil respon inflamai terhadap plak
pada gigi meningkat, ginggiva menjadi semakin membengkak dan berdarah ketika menyikat gigi.
Ginggivitis diperburuk oleh terjadinya perubahan hormonal pada wanita hamil yang disebut sebagai
pregnancy ginggivitis, meskipun secara histologi tidak berbeda dengan ginggivitis pada wanita tidak
hamil. Pregnancy ginggivitis merupakan manifestasi pada mulut paling banyak akibat kehamilan dan
pernah dilaporkan hingga 100% terjadi pada wanita hamil.
Pregnancy ginggivitis terlihat jelas setelah bulan kedua dari masa gestasi dan bertambah buruk
selama masa kehamilan dengan puncaknya pada umur kehamilan delapan bulan. Pada akhir bulan
masa
gestasi ginggivits biasanya sudah mulai berkurang dan dengan segera setelah melahirkan jaringan
ginggiva apabila dibandingkan akan terlihat sama dengan bulan kedua masa gestasi. Tetapi ini bukan
merupakan indikasi bahwa ginggiva sudah kembali sehat. Mengingat ba hwa ginggivitis dianggap
akibat pengaruh plak, namun Raber-Durlacher et al membuktikan bahwa pregnancy ginggivitis tidak
disebabkan oleh peningkatan plak gigi. Hal ini mungkin berhubungan dengan pengaruh kehamilan
pada jaringan ginggiva dengan ditemukannya reseptor estrogen dan progresteron., meskipun
mekanisme mengenai bagaimana secara tepatnya peningkatan hormon mempengaruhi inflamasi
ginggiva belum diketahui. Ini mungkin berhubungan dengan peningkatan pembuluh darah dan aliran
darah disamping perubahan s istem imun dan/atau perubahan metabolisme jaringan pengikat. (Pirie M,
et al, 2007)
Faktor penyebab timbulnya gingivitis pada masa kehamilan dapat dibagi 2 bagian, yaitu
penyebab primer dan sekunder. (Maulid G.A, 2008)
1. Penyebab primer
Iritasi lokal seperti plak merupakan penye bab primer gingivitis masa kehamilan sama halnya seperti
pada ibu yang tidak hamil, tetapi perubahan hormonal yang menyertai kehamilan dapat memperberat
reaksi peradangan pada gusi oleh iritasi lokal. Saat kehamilan terjadi perubahan dalam pemeliharaan
kebersihan gigi dan mulut yang bisa disebabkan oleh timbulnya perasaan mual, muntah, perasaan
takut ketika meng gosok gigi karena timbul perdarahan gusi atau ibu terlalu lelah dengan
kehamilannya sehingga ibu malas menggosok gigi. Keadaan ini dengan sendirinya akan menambah
penumpukan plak sehingga memperburuk keadaan.
2. Penyebab sekunder
Kehamilan merupakan keadan fisiologis yang menyebabkan perubahan keseimbangan hormonal,
terutama perubahan hormon estrogen dan progesteron. Peningkatan konsentrasi hormon estrogen dan
progesteron pada masa kehamilan mempunyai efek bervariasi pada jaringan, di antaranya pelebaran pe
mbuluh darah yang mengakibatkan bertambahnya aliran darah sehingga gusi menjadi lebih merah,
bengkak dan mudah mengalami perdarahan.
Hilangnya permukaan gigi, terutama melalui erosi akibat asam, terkait mual dan muntah yang
berulang selama kehamilan. Permukaan palatal gigi seri atas dan taring paling sering terkena (Gambar
4). Wanita umumnya mengeluh giginya lebih sensitif, yang merupakan akibat erosi sudah mencapai
lapisan dentin . Manajemen pada dasarnya adalah mencakup pencegahan dan penggunaan teratur
fluorida bilasan mulut, terutama pada wanita -wanita yang sering muntah. (Pirie M, et al, 2007) Selain
itu, para perempuan ini harus dianjurkan untuk menghindari menyikat gigi langsung setelah muntah
karena dapat memperburuk keadaan erosi gigi yaitu demineralisasi pada bagian permukaan.
3. Gigi Goyah
Meningkatnya mobilitas gigi telah terdeteksi dalam kehamilan bahkan pada periodontal wanita
sehat. Gigi seri atas paling goyah teutama ketika akhir bulan kehamilan. Mobilitas seperti ini mungkin
karena pergeseran mineral dalam lamina dura dan bukan modifikasi dari tulang alveolar. Penyakit
periodontal dan kerusakan jaringan ikat pada waktu hamil bisa memperburuk keadaan. (Pirie M, et al,
2007)
C. Pengaruh Penyakit Gigi dan Mulut pada Kehamilan
1. Prematur
- Ibu dengan penyakit perodontal dikaitkan dengan risiko persalinan prematur. (Suharso Edi, 2009)
- Bakteri mulut gram negatif menyebabkan respon inflamasi, respon inflamasi ini merangsang pros
taglandin dan sitokin yang akan memacu kontraksi uterus. (Suharso Edi, 2010)
- Penyakit periodontal yang timbul pada umur kehamilan 21 -24 minggu dapat menyebabkan kelahiran
preterm. (Marjorie, et al, 2001)
- Menurut penelitian dengan perawatan seperti plak kontrol, scaling, dan berkumur
dengan chlorhexidine 0,12%, menjaga kebersihan mulut, serta mengurangi timbulnya
plak setiap 2 sampai 3 minggu hingga melahirkan secara signifikan mengurangi
tingkat kelahiran prematur.
2. Berat Bayi Lahir Rendah (BBLR)
- Sebuah penelitian yang dilansir Annals of Periodontology di tahun 1998
mengungkap perempuan hamil dengan penyakit gusi yang kronis mempunyai risiko
tujuh kali lebih besar memiliki bayi lahir prematur. Riset lain menemukan sebanyak
77 persen ibu yang melahirkan bayi prematur menderita penyakit radang gusi.
(Abidin Boy, 2009)
- Mekanisme hampir sama dengan yang lahir prematur. Bakteri yang masuk melalui
gusi akan menyebar dan sampai di pembuluh darah ibu yang menuju ke janin.
Akibatnya pembuluh darah menjadi sempit sehingga asupan nutrisi pada fetal
tergannggu. Bisa sampai nekrosis dan timbul keguguran. (Abidin Boy, 2009)
- Penelitian membuktikan bahwa penyakit peri odontal merupakan faktor risiko
BBLR. (Shenoy R.P, 2009)
- Perawatan gigi secara benar tidak menimbulkan bahaya berat bayi lahir rendah
(Newham J.P. et al, 2009)
3. Preeklamsia
- Mekanisme terjadinya belum jelas
- Beberapa pendapat tentang mekanismenya sebagai beri kut: Infeksi
periodontalmenyebabkan inflamasi dan kerusakan vaskuler. Hal ini kan memicu
terjadinya kerusakan sel dalam plasenta sehingga timbullah preeklamsia. (Hasan T,
2009)
